To evaluate the influence of socioeconomic factors and provider characteristics on the use of intrapartum epidural anesthesia. ⋯ Use of intrapartum epidural analgesia varies considerably among sites and is associated with nulliparity, higher maternal age, and several nonclinical factors.

Case ReportsRuptured pelvic appendix diagnosed by transvaginal sonography.
Appendicitis in a pelvic appendix can be difficult to diagnose. Transvaginal sonography may help to visualize an inflamed pelvic appendix. ⋯ To our knowledge, this is the first case of appendicitis diagnosed with transvaginal sonography. Transvaginal sonography can delineate the features of an inflamed pelvic appendix and help to narrow the diagnostic possibilities in symptomatic women of childbearing age.

Abuse during pregnancy: effects on maternal complications and birth weight in adult and teenage women.
To determine the incidence of physical and sexual abuse in a sample of adult and teen pregnant women and to determine the effect of abuse on birth weight. ⋯ One in five teens and one in six adult women experienced abuse during pregnancy. Abuse is related to LBW and late entry into prenatal care. Abuse can be documented readily with a short abuse assessment screen and interventions then initiated.

Comparative StudyAbuse history and chronic pain in women: I. Prevalences of sexual abuse and physical abuse.
To compare the prevalences of childhood and adult physical and sexual abuse in women with chronic pelvic pain to those in women with chronic nonpelvic pain (headache) and pain-free women. ⋯ These results support a specific association between major sexual abuse and chronic pelvic pain and a more general association between physical abuse and chronic pain. Moreover, the global nature of the abuse histories of the women in the chronic pelvic pain group suggests that more rigorous studies of the relation between abuse history and chronic pelvic pain are needed.
