The authors reviewed the fetal outcomes for 30 pregnant patients who required hospitalization for burns that involved 11 to 97% of the total body area. A positive relationship was found between the percentage of maternal total body burn and maternal mortality, fetal mortality, and premature delivery. ⋯ After overcoming the immediate postburn period, a healthy-appearing term-size infant was likely to be born while the mother continued to receive intensive therapy. The ideal treatment to protect the fetus has not yet been determined.

Hypothalamic-pituitary-adrenocortical function after the dexamethasone-promethazine adhesion regimen.
Dexamethasone and promethazine are widely used as an antiadhesion regimen in infertility surgery. A major argument against the routine use of dexamethasone has been the concern that suppression of the hypothalamic-pituitary-adrenal axis may result. To investigate this concern, 25 healthy women undergoing surgery for infertility were administered dexamethasone and promethazine preoperatively, intraoperatively, and postoperatively for 48 hours. ⋯ Two women failed to achieve this rise, despite adequate hypoglycemia. Short-term suppression of the hypothalamic-pituitary-adrenal axis may occur after administration of the dexamethasone-promethazine regimen. Patients receiving this regimen should be carefully supervised during the postoperative period.

Preeclampsia associated with hemolysis, elevated liver enzymes, and low platelets--an obstetric emergency?
A study was undertaken of 27 patients with severe preeclampsia who had hemolysis, liver enzyme elevation, and thrombocytopenia as described by Weinstein. In addition to this triad, all patients exhibited the symptoms and signs of pregnancy-induced hypertension by which the diagnosis is usually established. These patients were admitted to the hospital for strict bed rest. ⋯ Maternal condition rapidly improved within 72 hours of delivery, and there was no persistence of thrombocytopenia or elevation of liver enzymes. Immediate delivery of preeclamptic patients who have thrombocytopenia and elevated liver enzymes may not be warranted. These findings suggest that the syndrome of hemolysis, elevated liver enzymes, and low platelets is not a separate entity, but merely a cluster of signs seen in some patients with hypertensive disorders in pregnancy.

Case ReportsManagement of dystocia caused by a large sacrococcygeal teratoma.
Sacrococcygeal teratoma is a rare cause of dystocia. Reported management strategies to date have yielded few viable infants. In this case report, the infant was partially delivered when extraction was halted. ⋯ The infant was subsequently delivered abdominally and three years later is normal. Despite absent thoracic cage movement during resuscitation, adequate oxygenation was possible. Increasing use of prenatal ultrasonography should allow early detection, and thus avoid such unanticipated dystocias.

Follow-up of patients with tubo-ovarian abscess(es) in association with salpingitis.
The medical records of 143 patients hospitalized with a diagnosis of salpingitis over a five-year period were reviewed. Ninety-three patients had salpingitis without clinical evidence of a tubo-ovarian abscess. Seven (7.5%) of these women had surgical treatment; five of the seven were found to have tubo-ovarian abscesses which had not been detected clinically. ⋯ Among women with a unilateral tubo-ovarian abscess, those who had a unilateral salpingo-oophorectomy had a higher pregnancy rate than those who received antibiotics alone. In this study, women with a tubo-ovarian abscess in association with salpingitis did not respond well to antibiotic treatment alone. This may be the most reliable way of distinguishing these patients from women with salpingitis alone or salpingitis in association with a tubo-ovarian inflammatory complex, who, as a group, did respond well to medical management alone.
