[Transobturator surgery for female urinary continence: from outside to inside or from inside to outside: a comparative anatomic study].
Transobturator route is now largely used for the positioning of the supporting sub uretral tape in the surgical treatment of female urinary incontinence. This operation can be done using the original technique from the outside to the inside or by inside to outside. Our anatomic study evaluates the specific dangers of each ⋯ The two techniques are not equivalent. There are less vascular and neurological risk using the original outside to inside technique.

Prospective clinical and urodynamic study evaluating modification of bladder sensation during sacral neuromodulation (SNM). ⋯ A significant correlation was observed between the two methods of clinical and urodynamic assessment. Our results suggest the use of the cystomanometric increase of bladder volume at B1 and B3 as selection criterion for candidates for SNM with non-neurological hyperactive bladder.

Pelvic laparoscopic surgery offers multiple advantages for patients, but requires uncomfortable, non-ergonomic positions for the surgeon. Only a remote operator (Robot "slave" reproducing the surgeon's movements performed on a "master" console situated away from the patient), like Da-Vinci or Zeus (Intuitive Surgical) can improve the laparoscopic surgeon's working conditions. The objective of this study, based on an ergonomic analysis and recording of a laparoscopic surgeon's movements, was to define the specifications for the manufacture of an operating seat with armrests, based on the surgeon's position using a remote operator. ⋯ The small amplitude of movements of the surgeon 's centre of pressure (< 45 cm2) confirmed that surgeons can be seated during laparoscopy. Recordings of the upper defined the elbow working zones and the need for 3D mobile armrests. Ergonomic analysis of posture defined the characteristics of the prototype. On the basis of these specifications, a prototype operating seat was developed. This prototype now needs to be validated or invalidated clinically.

[Ventriculo-ureteric shunt without associated nephrectomy for the treatment of hydrocephalus].
The authors report the case of a 51-year-old patient with hydrocephalus following posterior fossa surgery (cerebellar astrocytoma) treated by ventriculo-peritoneal shunt, then ventriculo-atrial shunt. After repeated valve revisions (diffuse peritoneal loculation, intracardiac thrombus responsible for dyspnoea and recurrent pulmonary embolism, shunt infections), another mode of shunting was required. The authors opted for ventriculo-ureteric shunt comprising ureteric reimplantation into a psoas bladder, without associated nephrectomy, which appears to constitute an alternative in the case of difficult surgical management of hydrocephalus or after failure of other modes of ventricular shunting.
