[Comparison of two methods of local anaesthesia prior to transrectal ultrasound-guided prostate biopsies].
To compare the analgesic efficacy of rectal administration of Lidocaïne gel with Lidocaïne periprostatic infiltration prior to transrectal ultrasound-guided prostate biopsies. ⋯ Rectal administration of Lidocaïne gel and infiltration of Lidocaïne lead to a comparable level of anaesthesia during prostatic biopsies procedure. However, the Lidocaïne gel, being both safe and simple, tends to maintain a better comfort of the patient 30 minutes after the end of the biopsies.

[Emphysematous cystitis complicated by bladder perforation: diagnosis and treatment of a rare case].
Emphysematous cystitis is a rare disease mainly encountered in poorly controlled diabetics, immunodepressed patients or patients with infravesical obstruction. The pathophysiology of this disease is characterized by the formation of carbon dioxide (CO2) present in the lumen and/or bladder wall, derived from bacterial fermentation of carbohydrates. ⋯ The authors report a case of emphysematous cystitis in a context of pelvic trauma and fracture of the pelvis, complicated by secondary bladder rupture. The diagnostic and therapeutic aspects are discussed.

Blunt trauma to the scrotum is increasingly frequent and is mainly due to motor vehicle accidents, especially with direct trauma from a motorbike petrol tank or falling astride a bicycle frame. The surgical exploration of these cases of trauma remains a controversial issue. However, according to the authors, the presence of haematocele on clinical examination justifies systematic early surgical exploration, which shortens the patient's length of hospital stay allowing more rapid return to work. Ultrasound is only really indicated in the case of scrotal trauma without haematocele, looking for rupture of the tunica albuginea or intratesticular haematoma.

To assess the value of scrotal ultrasound to guide the diagnosis of torsion of the spermatic cord. ⋯ As scrotal ultrasound gave false-negative results in 10% of cases and delayed surgery, we do not recommend this examination as an emergency investigation. Systematic surgical exploration of the scrotum in all cases of painful testis must be performed urgently without preoperative morphological assessment.
