Randomized Controlled Trial Comparative Study Clinical Trial[Incidence of postoperative shivering in relation to the time of sevoflurane discontinuation].
We aimed to determine whether early termination of sevoflurane administration lowers the incidence of postanesthetic shivering. ⋯ The important observation in this study was that the incidence of postoperative shivering in group A was lower than in group B and lower than the incidences reported in other similar studies.

Case Reports[Placement of a double-lumen tube using a 6 mm diameter fibro-bronchoscope and a Cook exchange catheter in a patient with unforeseen tracheal intubation difficulty].
The trachea of a 74-year-old man undergoing left pneumonectomy could not be intubated in two attempts after induction of anesthesia with midazolam, fentanyl, propofol and rocuronium. Difficult intubation had not been foreseen, but inspection through the laryngoscope revealed Cormack and Lehane grade IV conditions. Because a small-caliber fiberoptic bronchoscope was unavailable for intubation with a double-lumen endobronchial tube, we inserted a No. 9 orotracheal tube with a 6 mm bronchoscope as far as the left main bronchus. ⋯ As selective ventilation was required during surgery, a No. 11 Cook-type airway exchange catheter was inserted into the left main bronchus, the tracheal tube was removed, and was used to guide a No. 39F left double-lumen endobronchial tube through the bronchus. Insertion was uncomplicated and selective ventilation was satisfactory. The technique described is a new application for the Cook exchange catheter that allows selective bronchial in difficult cases when a small-caliber fiberoptic bronchoscope is unavailable.

[Sevoflurane anesthesia for magnetic resonance procedures in pediatrics].
To assess the efficacy and safety of sevoflurane anesthesia in children during magnetic resonance imaging procedures. ⋯ This study indicates that sevoflurane is safe and effective for sedating children, including newborn infants, who must undergo magnetic resonance imaging.

Randomized Controlled Trial Comparative Study Clinical Trial[Respiratory and hemodynamic effects of thoracic or lumbar epidural alfentanyl after thoracic surgery].
The aim of this study was to evaluate the effects on pulmonary function and hemodynamics of three different types of analgesia after thoracotomy. ⋯ Respiratory parameters were better after thoracic epidural analgesia with alfentanil and lidocaine than after the other analgesic techniques studied. Group L-A patients had greater need for rescue analgesia than did patients in the other groups.

Comparative Study Clinical Trial Controlled Clinical Trial[Effects of the alveolar recruitment manoeuver and PEEP on arterial oxygenation in anesthetized obese patients].
Diminished functional residual capacity and pulmonary collapse during general anesthesia lead to alterations in respiratory mechanics and gas exchange. Such phenomena are more pronounced in obese patients. We recently demonstrated the beneficial effects of the alveolar recruitment strategy on oxygenation in anesthetized patients of normal body mass index (BMI). The aim of the present study was to evaluate whether obese patients also benefit from the alveolar recruitment strategy and to determine the level of positive end-expiratory pressure (PEEP) that prevents recollapse in obese patients. ⋯ We conclude that the alveolar recruitment strategy was effective for increasing PaO2 in anesthetized patients, regardless of body mass. The oxygenation of obese patients receiving the higher level of PEEP was similar to that of non-obese patients.
