Observational StudyNon-operative management of blunt splenic injuries in a paediatric population: a 12-year experience.
Non-operative management (NOM) is now the primary treatment for blunt splenic injuries in children. Only one study has examined the use of NOM in a Scandinavian population. Thus, the purpose of this study is to report our experience in treating children with blunt splenic injuries with NOM at a Danish university hospital. ⋯ We demonstrated a high degree of success and safety of non-operative treatment in children with blunt splenic injury in a Scandinavian setting. Our results are comparable to international findings.

Good quality of oral anticoagulation treatment in general practice using international normalised ratio point of care testing.
Oral anticoagulation treatment (OACT) with warfarin is common in general practice. Increasingly, international normalised ratio (INR) point of care testing (POCT) is being used to manage patients. The aim of this study was to describe and analyse the quality of OACT with warfarin in general practice in the Capital Region of Denmark using INR POCT. ⋯ In our study most of the general practices using INR POCT in the management of patients in warfarin treatment provided good quality of care. Sampling interval and diagnostic coding were significantly correlated with treatment quality.

Spirometry utilisation among Danish adults initiating medication targeting obstructive lung disease.
This PhD thesis was written during my employment at the Research Unit of General Practice in Odense, University of Southern Denmark. It comprises an overview and three papers, all published or submitted for publication in international peer-reviewed scientific journals. ⋯ This study has shown a lack of spirometry testing among patients initiating medication targeting obstructive lung disease. This underuse of spirometry testing indicates a quality gap and increased focus of spirometry utilization is needed when patients initiate medication targeting obstructive lung disease. The variation reported in spirometry testing across patient and practice characteristics was most predominant with regard to increasing age among patients and doctors, the remaining variables only account for small variations. However identification of these variations can help guide general practitioners to identify patients at increased risk of not having spirometry performed and help target future interventions for primary care.

The effect of maternal exposure to psychosocial job strain on pregnancy outcomes and child development.
Psychological stress at work is a rising problem in Denmark. Nearly one third of the women reported in 2005 that they had difficulties completing their work tasks, and 17 % found that they had only limited or no influence on their work tasks. The corresponding numbers for 1987 were 18.3 % and 16 %, respectively. ⋯ Results did show effects in regards to allergic diseases, but as the study is the only one of its kind, other studies are needed to replicate the findings if to reconsider the recommendations for pregnant women. Future research could benefit of validation of the use of two single-item measures of job strain compared to fully validated multi-item instruments. A shift toward more "functional endpoints" as overweight and cognitive behavioural disorders in the children could be of relevance.

Observational StudyMore complications in patients with septic shock treated with dextran compared with crystalloids.
In recent years, the safety-profile of synthetic colloids has been questioned. The purpose of the present study was to elucidate the safety-profile of the colloid dextran-70 in relation to acute kidney injury (AKI) and death. ⋯ Patients in the dextran group had significantly more bleeding episodes, a higher need for CRRT and a lower urinary output than patients in the non-dextran group. Due to study design, it cannot be concluded that the use of dextran-70 is causally related to the development of AKI.
