Clinical Trial[A comparative analysis of haemodynamics and oxygen transport during xenon and nitrous oxide anaesthesia in endoscopic surgery in gynaecology].
A comparative analysis of haemodynamics and blood gases has been performed in patients during either low-flow anaesthesia with xenon and nitrous oxide and high-flow nitrous oxide anaesthesia. Ninety five patients scheduled for endoscopic surgery were included and divided into three groups: a combined low-flow anaesthesia (30 women), a low-flow nitrous oxide anaesthesia (30 women) and a high-flow nitrous oxide anaesthesia using a semi-closed circuit (35 women). The study has shown a stimulating effect of xenon on haemodynamics during carboxyperitoneum and Trendelenburg position, a better oxygen transport performance and a lower consumption of opioids in this group. Those favourable changes enable the optimization of the oxygen transport in endoscopic surgery in gynaecology, and reduce the contraindications to endoscopic surgery.

Case Reports[Anaesthesia and intensive care characteristics in HIV-infected pregnant patients receiving the antiretroviral therapy].
An analysis of features of the deliveries in HIV-infected patients with blood clotting failure, thrombocytopenia and anaemia developed during antiretroviral therapy was carried out. The article represents the anaesthesia care tactics and features of treating those patients during surgical delivery.

Case Reports[Analysis of transfusion related acute lung injuries after donor plasma transfusion in obstetrics].
The article presents a clinical investigation of two incidents of Transfusion Related Acute Lung Injury (TRALI) which have occurred after the fresh frozen plasma (FFP) transfusion in puerperant. According to evidence, when in timely diagnosed, the TRALI treatment is not complicated and lies in symptomatic lung oedema treatment. ⋯ Kulakov Research Center of Obstetrics, Gynaecology, and Perinatology is estimated as 1 event out of the 699.5 transfused FFP doses per year. Among of the most promising methods of reducing the risk of TRALI and other transfusion related complications are: reduction of donor blood transfusion, introduction of autohaemodonorship and education of physicians working in obstetrics.

[Prophylaxis and treatment of severe bleeding in obstetrics (draft guidelines)].
The article represents draft guidelines on severe bleeding in obstetrics which is the major reason of maternal mortality throughout the world. Criteria of estimation the physiological, pathological and massive blood loss are given. Algorithms of conservative and surgical treatment are presented, as well as major indications for administration of anti-fibrinolythics, infusion and transfusion treatment.

Clinical Trial[Prophylaxis of coagulopathic bleeding in pregnant patient of high-risk during delivery].
A clinic, laboratory and Doppler ultrasound study including women with a high risk of coagulopathic bleeding in delivery was carried out. When indicated, a coagulopathic bleeding prophylaxis was performed with administration of the tranexamic acid or with autotransfusion of the pre-stored fresh frozen plasma. Also, a reinfusion of washed red blood cells was performed using a Cell Saver. ⋯ Autotransfusion of plasma has favoured the recovery of blood clotting potential during and after the delivery. The obtained data have shown the safety of preparing the auto-plasma for both pregnant and foetus. The results of study prove the possibility of reducing the use of donor blood components with the aid of modern blood-saving technologies.
