[Etiology of postoperative bacteremias in an intensive care unit: an association with the level of procalcitonin].
The etiology of bacteremia determines the choice of adequate therapy for severe infections. The clinical manifestations of gram-negative and gram-positive bacterial infections are similar while biological markers may serve as a guide for the early diagnosis of the nature of a pathogen. The paper analyzes data on the rate and etiology of bacteremia in 1360 patients at the A. ⋯ In the gram-negative bacteremia group, plasma PCT levels were statistically significantly higher than in the gram-positive bacteremia group (5.40 and 0.86 ng/mg, respectively; p = 0.000000). On constructing a characteristic curve, the authors determined the PCT cut-off value of 5.2 ng/ml for the differential diagnosis of gram-negative and gram-positive bacteremia with the maximum (86%) specificity and more than 50% specificity. Thus, when patients with suspected infections have plasma PCT levels, the leading value of gram-negative microorganisms may be supposed just before obtaining blood culture results.

Randomized Controlled Trial Comparative Study[Comparative analysis of the efficiency and safety of different types of neuroaxial blocks during total hip replacement].
The paper presents the results of a prospective randomized study in 108 elderly patients undergoing total hip replacement under different neuroaxial blocks. The characteristics of sensory and motor blocks, hemodynamic changes, and a need for infusion and sympathomimetic support under epidural, spinal, and unilateral spinal anesthesia were comparatively assessed.

[Clinical significance of the parameters of a systemic inflammatory response, leukocytic intoxication index, and procalcitonin test in the diagnosis of pyodestructive pyelonephritis of pregnant women].
The clinical significance of the parameters of a systemic inflammatory response, leukocytic intoxication index, and procalcitonin test were studied in 63 patients with pyodestructive pyelonephritis of pregnant women. The blood concentration of procalcitonin was measured in 15 patients, by using the semiquantitative analysis. The authors revealed the high sensitivity and specificity of the method and the high prognostic value of positive results and the low frequency of false-positive results for procalcitonin and the leukocytic index of intoxication.

Clinical Trial[Efficiency of using gelofusine and voluven in acute normovolemic hemodilution during cardiosurgical interventions].
The aim of the study was to assess the use of gelofusine and voluven for acute normovolemic hemodilution at cardiac surgery under extracorporeal circulation (EC). Sixty-seven patients with coronary heart disease were examined. ⋯ The administration of gelofusine caused a more steady-state oxygen-transport function of the circulatory system. The use of the agent for acute normovolemic hemodilution at cardiac surgery under EC is more economically justified than that of volumen.

[The first experience of using sevorane in the Ikrutsk Cardiosurgical Center].
An anesthetic mode used during operations under extracorporeal circulation was comparatively analyzed in children operated on for congenital heart diseases. The preference of inhalational anesthesia using the anesthetic sevorane was revealed. The use of the anesthetic reduced the need for opioid analgesics and activated patients in the early postoperative period.
