Case Reports[Long-term remission after erlotinib therapy in an elderly patient with advanced non-small-cell lung cancer. Case report and conclusions for clinical practice].
Small molecule tyrosine kinase inhibitors (TKIs) of epidermal growth factor receptor (EGFR) - gefitinib and erlotinib - have recently been used as a therapeutic option in advanced non-small cell lung cancer (NSCLC) patients relapsing after first- or second-line chemotherapy. We report here a case of long-term remission in an elderly, non-smoking woman with advanced NSCLC after chemotherapy failure, who was selected for erlotinib therapy using demographic and clinical criteria. Based on this example and on the literature data we discuss the need for careful patient selection for this new therapeutic method.

Controlled Clinical Trial[Pulmonary rehabilitation in patients with idiopathic pulmonary fibrosis with inspiratory muscle training].
Evaluation of effectiveness of pulmonary rehabilitation in patients with idiopathic pulmonary fibrosis (IPF) has not yet been presented in medical literature. The objective of the study is to analyze the influence of inspiratory muscle training on dyspnea (oxygen cost diagram [OCD], baseline dyspnea index [BDI]), quality of life (SF-36), results of 6 MWT (distance, dyspnea in Borg's scale), maximal inspiratory pressure (MIP), and lung function tests (IC, TLC, VC, FEV1, DLCO(SB), DLCO/VA) in patients with IPF. ⋯ Adding inspiratory muscle training increases effectiveness of pulmonary rehabilitation in IPF patients.

Obstructive sleep apnea (OSA) is a well-recognized risk factor of cardiovascular disorders and is related to metabolic syndrome. The aim of this study was to evaluate the effect of BMI and AHI/RDI on metabolic disturbances in patients suspected of OSA. ⋯ Our findings support the results of previous studies showing the influence of OSA alone on metabolic disturbances. However, BMI has major impact on metabolic variables.

The aim of this study was to assess results of 6MWT in non-selected group of obstructive sleep apnoea (OSA) patients. In healthy adult subjects 6-minute walking distance (6MWD) range from 400 to 700 m. Obesity, the main symptom of OSA, is one of the factors associated with reduction of 6MWD (another common factors: older age, shorter height, female sex, pulmonary, cardiovascular and musculoskeletal diseases). ⋯ BMI, female sex, arterial hypertension and lower FVC (% of predicted) were related to shorter 6-minute walking distance in OSA patients.

[Ultrasound guided transbronchial needle aspiration as a diagnostic tool for lung cancer and sarcoidosis].
The aim of the study was to assess the diagnostic yield of ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) in mediastinal or hilar adenopathy in: 1) staging of non-small cell lung cancer (NSCLC) (97); 2) other malignant neoplasms including: small cell lung cancer (SCLC), metastatic neoplasms and Hodgkin's disease (16); 3) NSCLC recurrence (7); 4) sarcoidosis and other non-malignant diseases (29). ⋯ The diagnostic value of EBUS-TBNA is very high in lung cancer, NSCLC staging and sarcoidosis.
