The Cleveland Clinic experience with supraclavicular and popliteal ambulatory nerve catheters.
Continuous peripheral nerve blocks (CPNB) are commonly used for intraoperative and postoperative analgesia. Our study aimed at describing our experience with ambulatory peripheral nerve catheters. After Institutional Review Board approval, records for all patients discharged with supraclavicular or popliteal catheters between January 1, 2009 and December 31, 2011 were reviewed. ⋯ Forty-two possible complications were identified: 13 infectious, 23 pharmacologic, and 6 labeled as other. Two patients had retained catheters, 2 had catheter leakage, and 2 had shortness of breath. Our study showed that prolonged use of ambulatory catheters for a median period of 5 days did not lead to an increased incidence of complications.

Use of the mechanical leech for successful zone I replantation.
Replantation of zone I finger injuries remains a challenge, particularly if the fingertip was previously scarred or atrophied, which makes it difficult to secure a suitable vein at the amputation site. In cases of artery-only anastomosis, we propose using a mechanical leech technique to maintain sufficient venous outflow until the internal circulation regenerates. We applied this procedure to eight patients who had zone 1 amputations without veins that were suitable for anastomosis. ⋯ As there were no veins available, we cut a branch of the central artery and anastomosed it with a 24-gauge angioneedle, which served as a conduit for venous drainage. The overall survival rate for zone I replantation using mechanical leech was 87.5% and the average time to maintain the mechanical leech was 5 days. The mechanical leech technique may serve as an alternative option for the management of venous congestion when no viable veins are available.

Cross-sectional investigation of HEMS activities in Europe: a feasibility study.
To gather information on helicopter emergency medical services (HEMSs) activities across Europe. ⋯ The study proved feasible, establishing an embryonic network of European HEMS. The participation rate was low and limits the generalizability of the results. Fortunately, because of its cross-sectional characteristics and the handy availability of the web platform, the study is easily repeatable with an enhanced network.

Temperament and character personality profile and illness-related stress in central serous chorioretinopathy.
Psychological stress is a risk factor as well as a consequence of central serous chorioretinopathy (CSC). Impulsiveness, overachievement, emotional instability, and hard-driving competitiveness have been discussed as personality features in CSC patients. We investigated 57 consecutive CSC patients and 57 age- and gender-matched controls by means of the Symptom Checklist 90-R and the Temperament and Character Inventory. ⋯ Implicating competitiveness, hostility and emotional detachment, lower level of cooperativeness, and reward dependence support the existence of specific aspects of type A behaviour in CSC patients. Low perceived social support and loss of control may explain the significant contribution of this personality dimension to illness-related work stress. Treatment of CSC should thus incorporate psychoeducation about factors contributing to illness-related stress.

Delay analysis of networked control systems based on 100 M switched Ethernet.
For the delay may degrade the performance of networked control systems, networked control systems based on 100 M switched Ethernet are proposed in this paper. According to the working principle of Ethernet switch, the formulas of the upper bound delay of the single-level switched Ethernet and the multiple-level switched Ethernet are deduced by the timing diagram method, and the values of the upper bound delay are also given. The key factors that influence the upper bound delay of switched Ethernet are analyzed; then, the characteristics of the upper bound delay are presented, which show that the delay induced by the single-level 100 M switched Ethernet has little effect on the performance of control systems, while the delay induced by the multiple-level 100 M switched Ethernet may meet the time requirements of all classes of control systems if the numbers of levels and the numbers of nodes connecting to switches are set properly. Finally, the performance of networked control systems is simulated by TrueTime, and the results further show the feasibility and superiority of 100 M switched Ethernet based networked control systems without modification of the network protocols.
