The Court of Protection is the specialist court that makes decisions in relation to people who lack decision-making capacity. In this article, Richard Griffith considers the powers of the Court of Protection and discusses when it might be necessary for a district nurse to seek an order from the court in relation to the care or treatment of a patient.

After a burn injury, skin loses its protective properties, leaving the body open to a number of complications. The gold standard treatment for burn wounds is autologous skin grafting; however, this may not always be an option, due to insufficient amounts of donor skin. Wound dressings can be used as a primary treatment in those patients who are unsuitable for skin grafting. ⋯ There is currently no gold standard of burns dressing that is universally accepted. However, hydrogels have been shown to exhibit a great number of beneficial properties and are the most favourable for use in burns patients. We discuss the use of hydrogel dressings, including their properties and clinical application.

Breathlessness or dyspnoea is a subjective experience that can be described as an unpleasant or uncomfortable awareness of breathing. It is a subjective experience for patients and often they learn to adapt to the limitations caused by their condition, which makes their breathlessness less apparent to others. ⋯ Chronic refractory breathlessness is defined as breathlessness at rest or on minimal exertion that will persist chronically despite optimal treatment of the underlying causative factors. The role of the community nurse in managing the breathless patient should involve differentiating between different types of breathlessness and knowing how to effectively manage it in a holistic manner.
