The International Society of Hypertension and the World Health Organisation have recently published new guidelines for the management of hypertension. Epidemiologic studies have shown that management of hypertension still remains unsatisfactory. ⋯ The target blood pressure under treatment must be less than 140/90 mmHg, and even lower for diabetic or chronic renal failure patients. In parallel, all cardio-vascular risk factors have to be corrected to decrease the global cardio-vascular risk.

Chronic headaches due to intracranial hypo- or hypertension (IHS codes 7.2 and 7.1) may be difficult to diagnose. In this article, we review their principal clinical characteristics, etiologies and therapies. Intracranial hypotension may be caused by CSF linkage, e.g. after lumbar puncture. ⋯ Increased opening pressure of the CSF will confirm the diagnosis. Etiologies such as cerebral venous thrombosis, have to be excluded by adequate imaging methods. In both hypo- and hypertension syndromes, various therapeutic strategies have been proposed.

Review[Pharma-clinics. The drug of the month. Aceclofenac (Biofenac)].
Aceclofenac, a phenylacetic acid derivative, is a new nonsteroidal anti-inflammatory drug (NSAID). It is indicated in the symptomatic treatment of pain and inflammatory or degenerative arthropathies: osteoarthritis, rheumatoid arthritis, ankylosing spondylitis, abarticular inflammations, posttrauma or postoperative inflammations. ⋯ Its clinical efficacy seems to be similar to that of other NSAIDs, but its safety profile, especially gastro-intestinal tolerance, appears to be better. Finally, in vitro studies suggested that aceclofenac may exert positive effect on cartilage matrix metabolism.

Review[Pharma clinics. How I treat...or preventing pain with "patches"].
There exists two distinct types of patches aiming at increasing the pain threshold either systematically or locally. The first type of analgesic patch is a real transdermal delivery system releasing fentanyl. Such potent opioid drug exerts a systemic effect. The other type of patch associates lidocain and prilocain to induce a local anesthesia of the skin without inducing a systemic effect.
