Spinal anaesthesia is the method of choice for elective caesarean delivery. Levobupivacaine may produce a sensory and motor block that is different from that produced by bupivacaine, the most popular local anaesthetic for parturients undergoing caesarean section (CS). We present a case of unexpectedly prolonged spinal anaesthesia following a successful spinal block with levobupivacaine. There was no evidence of any neurological injury in this patient during injection.

Comparative Study
A comparison of the effects of desflurane and isoflurane on rat pulmonary parenchyme histopathology and malondialdehyde levels.
To evaluate the lipid peroxidation and pulmonary histopathology after desflurane and isoflurane anaesthesia in rats. ⋯ Desflurane significantly increased pulmonary inflammation more than isoflurane in rat pulmonary parenchyma that indicates an inflammatory response. Besides, it was determined that the significantly higher malondialdehyde levels in both the desflurane and the isoflurane groups resulted in an increase in the membrane lipid peroxidation via volatile anaesthetics.

Comparison of upper lip bite test with Mallampati test in the prediction of difficult intubation at a tertiary care hospital of Pakistan.
To determine the accuracy of the Upper lip bite test and Mallampati test in predicting difficult endotracheal intubation. ⋯ Upper lip bite test is an acceptable alternative as a single test to predict difficult intubation in addition to other tests of airway assessment for the prediction of difficult intubation.

Subclavian steal syndrome (SSS) is a rare condition. It results from subclavian artery (SA) stenosis proximal to the origin of the vertebral artery. It is characterized by cerebral ischaemia with associated symptoms of vertebrobasilar hypoperfusion and/or symptoms of brainstem or arm ischaemia. ⋯ Treatment is aimed at restoring permanent antegrade blood flow to the affected vertebral artery. This abolishes vertebral basilar symptoms and other manifestations of SSS. Several modalities exist, however surgical correction is the treatment of choice.

To observe the safety of trans-jugular pressure-monitoring catheter insertion at a tertiary care teaching hospital in Rawalpindi, Pakistan. ⋯ Insertion of trans-jugular pressure monitoring catheters during cardiac surgery for congenital heart disease is a safe and reliable technique.
