Case ReportsAbruptio placenta following sexual intercourse: case report.
This is a case of Abruptio Placenta resulting from sexual intercourse. Pelvic Examination revealed active bleeding per vaginam. ⋯ The foetus membranes were ruptured and labour progressed. A still born male baby was delivered.

Comparative StudySpinal and general anaesthesia for emergency caesarean section: effects on neonatal Apgar score and maternal haematocrit.
This is a prospective comparative study to determine the effect of the type of anaesthesia used for emergency Caesarean section on neonatal and maternal outcome. The patients were recruited into the study after being given either general or spinal anaesthesia. Neonatal outcome was assessed using Apgar scores and need for respiratory assistance at birth. ⋯ Apgar scores at 1 and 5 minutes were found to be significantly lower for the general anaesthesia group (GA) than the spinal anaesthesia group (SA) and need for respiratory assistance was greater for the GA group. Difference between pre- and post-operative PCV and need for blood transfusion were also significantly greater in the GA group. This study confirms that the current practice of spinal anaesthesia for Caesarean section in the Lagos University Teaching Hospital is a good one, but further studies need to be done to assess other outcome variables.

Case ReportsReexpansion pulmonary oedema as a complication of pleural drainage.
One hundred and one (6%) of 1,678 patient studied had bilateral reexpansion pulmonary oedema(RPO). On the whole, one thousand, seven hundred and seventy nine (1,779) pleural spaces were studied, fifteen pleural spaces (0.8%), with mean age of 23 +/- 4.5 years had RPO. Among these 15 patients with RPO, the mean period of lung collapse before pneumothorax (PThx) was evacuated was 31.8 +/- 21.8 days and for hydrothorax (HThx) was 31.3 +/- 30.1 days; for 15 patients without RPO (controls), matched for age and sex, the mean period of lung collapse before CTTD was 7.5 +/- 4.1 days and 5.4 +/- 1.3 days respectively for PThx and HThx. ⋯ We conclude that prevention of RPO is the desired goal in the management of pleural effusion or Pneumothorax. RPO is commonest among young patients who have had lung collapse for 7 or more days. In these circumstances RPO is prevented, its incidence and severity reduced by methods of gradual evacuation of PThx or pleural fluid drainage.

Anaesthesia as a career--the influence of undergraduate education in a Nigerian Medical School.
Sixty-seven final year medical students of the Obafemi Awolowo University, Ile-Ife, Nigeria were questioned to find out the influence of undergraduate education on their choice of future career. Eighty per cent of them found anaesthesia interesting and important but none would pick anaesthesia as a first choice for future career. ⋯ The students made several suggestions on how to improve undergraduate medical training. The author also suggests measures for general improvement in all aspects of anaesthetic practice in Nigeria with a view to attracting more residents into this specialty.

Medical admission into the intensive care unit (ICU) of the Lagos University Teaching Hospital.
The paper presents patterns of medical admissions into the intensive care unit of the Lagos University Teaching Hospital (LUTH) over an eight-year period from September 1990 to August 1998. Medical admission constituted 15% out of which 1% received surgery for medical complications. Patients with neurological diseases recorded the highest number of admission most of them being for tetanus. ⋯ The overall mortality rate was 69% and it was observed that the ages of the patients did not appear to affect the outcome except in patients who were admitted for myocardial infarction and cardiogenic shock. This study emphasizes the need to evolve a system of health evaluation of predicting the survival index of individual patients. There is an urgent need for proper training, motivation of staff and maintenance of equipment used in the ICU.
