Short-term trends in heart failure-related hospitalizations in a high-risk state.
We sought to determine whether there are signs of improvement in the rates of heart failure (HF) hospitalizations given the recent reports of improvement in national trends. ⋯ The overall rate of HF hospitalization declined during the period surveyed, but the persistent disproportionate involvement of blacks with evidence of worsening among younger black men, requires close attention.

Because of high rates of violent gun-related injuries seen in emergency department (EDs), the ED has become involved in prevention violence intervention. The purpose of the study was to determine the relation between access to guns and the risk of violence-related injuries in youth and young adults. ⋯ There was no difference between the two subject groups in terms of their being victims of violence and the rate of gun accessibility. There was, however, a difference in their attitudes toward guns.

Long-term impact of environmental public health disaster on health system performance: experiences from the Graniteville, South Carolina chlorine spill.
In the aftermath of an environmental public health disaster (EPHD) a healthcare system may be the least equipped entity to respond. Preventable visits for ambulatory care-sensitive conditions (ACSCs) may be used as a population-based indicator to monitor health system access postdisaster. The objective of this study was to examine whether ACSC rates among vulnerable subpopulations are sensitive to the impact of a disaster. ⋯ Results revealed that an increased demand on the health system altered health services delivery for vulnerable populations directly affected by a disaster. Preventable visits for ACSCs may advance public health practice by identifying healthcare disparities during disaster recovery.

Multicenter StudyUsing appendiceal perforation rates to measure impact of a disaster on healthcare system effectiveness.
To understand baseline inequities in appendiceal perforation rates and the impact of hurricane destruction on the healthcare system with respect to perforation rates and racial disparities. ⋯ Health professionals and hospital systems were able to maintain effective levels of care before and after Hurricane Katrina; however, perforation rates in African Americans suggest ongoing racial disparities during disasters.

Burn disaster preparedness and the southern region of the United States.
Disasters with significant numbers of burn-injured patients create incredible challenges for disaster planners. Although not unique to burn care, high-intensity areas of specialty such as burns, pediatrics, and trauma quickly become scarce resources in a disaster. All disasters are local, but regional support is critical in burn disaster planning. ⋯ This article also explores planning and preparedness developments and describes options to improve our efforts, including training and education. It is incumbent upon everyone in the healthcare profession to become comfortable managing burn-injured patients until the patients can be moved to a burn center. Understanding the regional capacity, capability, and when a surge of patients may require the practice of altered standards of care is essential for those involved in medical disaster preparedness.
