ReviewColloid oncotic pressure as a guide for the anesthesiologist in directing fluid therapy.
One useful but underused parameter of fluid replacement is colloid oncotic pressure. Colloid oncotic pressure (COP) is one of the Starling forces that maintain a balance between intravascular and extravascular fluid. ⋯ Systemic transcapillary fluid transport, however, is governed significantly by COP, and serial measurements of COP can serve as useful guides for colloid replacement. In this paper we present instances in which COP determinations were found to be clinically helpful, and discuss colloid replacement during surgery.

Case ReportsSeptic shock and acute respiratory distress syndrome after salpingitis caused by Streptococcus pyogenes group A.
A 32-year-old woman with acute salpingitis had signs and symptoms of sepsis, with hypotension, renal failure, acute respiratory distress syndrome, and disseminated intravascular coagulation. Streptococcus pyogenes group A was grown from blood cultures taken at the onset of illness, and salpingitis was confirmed at laparotomy. The patient recovered after appropriate antimicrobial and intensive supportive therapy.

The weight increase that is an often reported side effect of psychotropic drug use has implications for health risks as well as medication noncompliance. In this article I review the literature on the association between the use of several classes of psychotropic drugs and weight gain, discuss the possible mechanisms for this effect, and offer some practical approaches for reducing the risk of drug-induced weight gain, including the identification of a group of patients "at risk."
