Randomized Controlled Trial Clinical TrialNonresponders to clopidogrel therapy among Indian patients undergoing elective/adhoc angioplasty.
Clopidogrel has become the standard antiplatelet drug along with aspirin in patients undergoing coronary angioplasty; however, data regarding the nonresponse rate to clopidogrel therapy in Indian patients are limited. ⋯ Clopidogrel nonresponse is prevalent among Indian patients, and there is wide interpatient variability in platelet inhibition among individual patients. However, the clinical implications of these findings need to be substantiated in larger studies with clinical end points.

Extrinsic esophageal compression leading to dysphagia is a recognized but uncommon presentation of thoracic aortic aneurysms. This has been referred to as dysphagia aortica, and is a late presentation of large thoracic aneurysms. ⋯ Eventually, thoracic-abdominal aortic aneurysm causing compression of the esophagus was discovered. When dealing with patients who present with dysphagia and on and off chest pain and backache if no other cause is discovered, a thoracic aortic aneurysm should form part of the differential diagnosis, as this is potentially curable, and delay in diagnosis and treatment predisposes to rupture and death.

Cardiac involvement with Salmonella infection is well known and electrocardiographic changes bear a direct relationship to prognosis. We present here a case with ECG changes mimicking an acute myocardial infarction in salmonella infection.

Clinical correlation of multiple biomarkers for risk assessment in patients with acute coronary syndrome.
Biochemical markers are useful for the prediction of future cardiovascular events in patients with non-ST-segment elevation acute coronary syndrome (ACS). The independent as well as the combined prognostic value of elevated troponin-T, high-sensitivity C-reactive protein (hs-CRP), and N-terminal pro-brain natriuretic peptide (NT-pro-BNP) on the Thrombolysis In Myocardial Infarction (TIMI) risk score and on the short-term prognosis were evaluated in a cohort of ACS patients. ⋯ The use of NT-pro-BNP, hs-CRP, and troponin-T in combination appears to add critical prognostic insight to the assessment of patients with ACS.
