A 70-year-old man underwent coronary artery bypass graft complicated postoperatively by visual loss. The diagnosis was nonarteritic anterior ischemic optic neuropathy. Possible predisposing factors in this patient were hypotension, anemia, a "disk at risk," and internal carotid artery stenosis. In the postoperative setting, the erythrocyte sedimentation rate may be elevated, as it was in this case and does not by itself suggest a diagnosis of giant cell arteritis.

Biography Historical ArticleRubens Peale's spectacles: an optical illusion?
The painting Rubens Peale with a Geranium (1801), by Rembrandt Peale, has earned a reputation as a masterpiece of early American portraiture. In recent years the painting has also been the source of controversy, because Rubens was depicted with two pairs of eyeglasses at a time when most people would not have been portrayed with spectacles at all. Scholars of American art history and ophthalmology have studied the painting and have promoted various theories for this peculiarity. A combined study of the painting, historical documentation, and optical effects in the painting, however, sheds new light on the answer to this mystery.

ReviewNormal and pathological mechanisms in retinal vascular development.
Angiogenesis is a complex biologic process that occurs normally in development and in turnover and remodeling of mature vascular networks. Pathological angiogenesis and neovascularization occur in association with retinal and ocular ischemic diseases, in retinopathy of prematurity and other developmental disorders, and in tumor growth and metastasis. We describe current understanding of cellular and molecular mechanisms of retinal vascular development, highlighting aspects that relate to eye diseases, that provide sites of therapeutic intervention in ophthalmology and that are potential avenues for research.
