A 18-year-old female complained of a dyspneic sensation and involuntary movements in the epigastrium. On physical examination, fine rhythmic movements in the epigastrium were observed on each inspiration with a rate of 5-10 per breath. Arterial blood gas analysis was normal. ⋯ In two of them, the spontaneous respiration was superimposed on a high-frequency wave throughout the whole respiratory cycle while in one case the high-frequency wave developed only in the inspiratory phase of spontaneous respiration. In the present case the respiratory flow pattern was similar to the last mentioned and the EMG-analysis of respiratory muscles confirmed that the flutter waves were driven by intercostal muscles and diaphragm. Furthermore, synchronization of activities of the two respiratory muscles suggests that the syndrome is of central origin.

Case Reports[A case of obstructive sleep apnea syndrome remarkably improved by gastric restriction surgery].
A 23-year-man with morbid obesity and obstructive sleep apnea syndrome (OSAS) was admitted. He was 170 cm in height and 170 kg in weight. He underwent dietary treatment several times, but his weight returned to its original level, or even higher, within a short period. ⋯ The postoperative findings revealed a dramatic body weight reduction. At the same time, the results of apnea and oxygen desaturation were remarkably improved too. These results indicate that weight reduction surgery is a definitely effective treatment for morbid obesity associated with OSAS.

Case Reports[Three cases of pneumothorax due to lung cancer].
Spontaneous pneumothorax due to primary lung cancers are increasingly reported as of lung cancer incidence increased. We experienced one lung cancer case with pneumothorax as a primary symptom and two who developed pneumothorax after chemotherapy, among 238 cases of primary lung cancer. In the literature, 76 cases of pneumothorax due to primary lung cancers are described. The mechanism of pneumothorax is discussed.

Case Reports[Tracheal tumor resection and reconstruction surgery].
Ninety-six tracheal tumor patients were treated surgically. Tracheal resection was performed in 79 patients with 11 primary tracheal tumors (nine with adenoid cystic carcinoma), and 68 secondary tumors involving the trachea (62 with thyroid carcinoma). Resection of the trachea plus carina was performed in 16 patients with 10 primary pulmonary carcinomas, 4 adenoid cystic carcinomas and 2 varied tumors. ⋯ Postoperative radiotherapy appeared to be effective in obviating local recurrence in patients with adenoid cystic carcinoma of the trachea. In patients with thyroid carcinoma invading the trachea, of the 29 patients eligible for 5-year follow-up, 19 were still alive, and of the 10 patients eligible for 10-year follow up, seven were still alive. However, in patients with thyroid carcinoma remaining after tracheal resection, 10-year survival was rare.

Case Reports[Two cases of spontaneous pneumomediastinum due to Xiao-lin Temple boxing vocal exercise].
Two cases of spontaneous pneumomediastinum and subcutaneous emphysema were reported. The first cases was a 21-year-old man who had a past history of recurrent spontaneous pneumothorax. The second was a healthy 20-year-old man. ⋯ The chest X-ray films revealed intramediastinal air and subcutaneous emphysema in both cases. We diagnosed these patients as spontaneous pneumomediastinum because of no underlying disease. This is a report of rare cases of spontaneous pneumomediastinum occurring at the same time, and one of the mechanism causing them was considered to be a sudden increase in intrathoracic pressure due to the vocal exercise of chinese boxing.
