Case Reports[A case of survival for 50 months after resection of cerebellar metastasis of squamous cell lung carcinoma].
The patient was a 64-year-old male with squamous cell carcinoma of the left lung, associated with intrapulmonary metastasis. Eleven months after lobectomy, a single metastasis occurred in the left cerebellum. ⋯ Fifty months after the resection, the patient is still alive without recurrence. Based on the present case, we consider that, in the case of single brain metastasis, surgical therapy is desirable because long-term survival can be expected with surgical resection even for a cerebellar metastasis.

[Significance of systemic inflammatory response syndrome at cardiopulmonary bypass].
Systemic Inflammatory Response Syndrome (SIRS) is a new concept of entry criteria for sepsis. This concept, when applied to area of Multiple Organ Failure (MOF), is considered to be a preparatory state for MOF. To study the significance of SIRS state at cardiac surgery, we measured the body temperature, white blood cell count, respiratory rate and heart rate of 18 patients who underwent elective cardiac surgery, from the 1st post-operative day to the 7th post-operative day. ⋯ Serum levels of IL-8 at SIRS state were revealed statistically higher than at non-SIRS case. Duration of SIRS state was related to CPB time and serum levels of IL-6 and IL-8 just after CPB. We concluded that SIRS state is an indication for anti-cytokine therapy to prevent MOF, and it is important to shorten CPB time in order to decrease the duration of SIRS.

Case Reports[Isolated rupture of the ventricular septum caused by nonpenetrating trauma--a case report of operation in acute stage].
We experienced a case of the isolated ventricular septal defect due to nonpenetrating chest trauma. The patient was 22-year-old male who hit his chest with a steering wheel on car accident. The echocardiogram examined on the next day revealed a ventricular septal defect. ⋯ On cardiopulmonary bypass the ventricular perforation (35 x 35 mm size) located in the muscular portion near the apex was closed with a PTFE and equestrian pericardial patch. Postoperative cardiopulmonary function could improve enough to wean from IABP and respiratory support. However, the patient died on the fifth postoperative day of severe hepatic failure due to profound shock persisted after the accident.

Case Reports[A successful surgical case of descending necrotizing mediastinitis with fistula formation to the right main bronchus].
Descending necrotizing mediastinitis (DNM) is extremely rare and one of the most lethal forms of mediastinitis, even in the era of antibiotics. We have recently treated a 65-year-old man who was diagnosed as having a fistula to the right main bronchus caused by DNM secondary to a peritonsillar abscess. ⋯ Postoperative course was uneventful. This is the second known reported case of a successful operation for DNM with bronchial fistula.

Case Reports[A case report on a reconstructive operation of a traumatic tricuspid regurgitation with a congenital defect of the left pericardium].
We reported a successful tricuspid valve replacement in a 58-years old man, who had easy fatiguability after 14 years of a blunt chest trauma. The preoperative examination revealed a marked cardiomegaly with deformation of both ventricles and grade 4 tricuspid regurgitation caused by the prolapse of the anterior leaflet. The operative inspection revealed a left pericardial defect with a diameter of 10 cm and a torn anterior papillary muscle. ⋯ Atrioventricular conduction was preserved. The tricuspid valve was not resected to preserve the ventricular function. The patient recovered his own activity.
