Classification of chronic orofacial pain: applicability of chronic headache criteria.
The aim of this study was to test the temporal definitions of chronic daily headache (CDH) in an orofacial pain population and examine the features of the so-defined chronic orofacial pain (COFP). ⋯ This study provides a base for the clear definition of COFP that has the distinctive advantage of being identical to that used in headache. Additionally, our results clearly show that COFP should be regarded as a temporal definition and not a diagnosis.

Case ReportsOn the use of prothrombin complex concentrate in patients with coagulopathy requiring tooth extraction.
In patients on high-level anticoagulant therapy (prothrombin time-international normalized ratio [PT-INR] ≥ 4.5), surgical procedures can be carried out with bridging therapy using heparin. However, surgical treatment options are severely limited in patients on high-level anticoagulant therapy and who have heparin-induced thrombocytopenia (HIT), as heparin use is contraindicated. We performed tooth extraction using prothrombin complex concentrate (PCC) in 2 HIT patients on high-level anticoagulation therapy (PT-INR ≥ 4.5). ⋯ Tooth extraction was then performed and sufficient local hemostasis was achieved. At 3 hours after tooth extraction, PT-INR was 2.0 or higher and later increased to 4.0 or higher, but postoperative bleeding was mostly absent. When performing tooth extraction in HIT patients on high-level anticoagulant therapy, favorable hemostatic management was achieved through sufficient local hemostasis and transient warfarin reversal using PCC.

Awareness of complications of oral piercing in a group of adolescents and young South African adults.
Intraoral piercing as a body art has been gaining popularity amongst adolescents and young adults; however there is limited data on the awareness to complications associated with it. This study investigated the awareness of complications of oral piercing among a group of adolescents and young South African adults with intraoral piercings. ⋯ The general lack of awareness of complications related to oral piercing needs to be addressed by the dental community.

Case ReportsPseudoaneurysm of the facial artery as a late complication of bilateral sagittal split osteotomy and facial trauma.
Pseudoaneurysms, or arteriovenous fistulas, of the extracranial arterial system, particularly the facial artery, are rare. Its occurrence after reconstructive facial surgery has been reported rarely in the literature. In this article we describe a rare case of pseudoaneurysm developing over 4 weeks after an uneventful Le Fort I maxillary osteotomy, sagittal split osteotomy, and advancement genioplasty. ⋯ The patient tolerated the procedure well and a repeat angiogram demonstrated no further evidence of aneurysm, arteriovenous malformation, vasospasm, or feeding branches to the fistula. The patient made an uneventful recovery and was discharged the day after the procedure. In this article, we review the anatomy of the extracranial arterial system of the head and neck, discuss the pathogenesis and clinical presentation of pseudoaneurysm, and present diagnostic imaging and treatment options for pseudoaneurysms of the face.

ReviewRelationship between bruxism and temporomandibular disorders: a systematic review of literature from 1998 to 2008.
The present paper aims to systematically review the literature on the temporomandibular disorders (TMD)-bruxism relationship published from 1998 to 2008. ⋯ Investigations based on self-report or clinical bruxism diagnosis showed a positive association with TMD pain, but they are characterized by some potential bias and confounders at the diagnostic level (eg, pain as a criterion for bruxism diagnosis). Studies based on more quantitative and specific methods to diagnose bruxism showed much lower association with TMD symptoms. Anterior tooth wear was not found to be a major risk factor for TMD. Experimental sustained jaw clenching may provoke acute muscle tenderness, but it is not analogous to myogenous TMD pain, so such studies may not help clarify the clinical relationship between bruxism and TMD.
