Many health care providers lack cultural competency and are unaware of how to meet the needs of the culturally diverse patient. All health care providers, and particularly nurses and nurse practitioners who are on the front lines of patient care, must be skilled in providing culturally appropriate and competent care.

In Vermont, too much was spent on institutional care and not enough on home and community-based services. One study, written for Vermont's General Assembly, suggested that Vermont should work toward a gradual shift in spending with more spent on home care services and less on nursing home care.
