Left ventricular ejection fraction (LVEF) is accepted as an important prognostic marker in patients (pts) with implantable cardioverter-defibrillators (ICD). The impact of this therapeutic approach in the survival of pts with life-threatening arrhythmias and severe left ventricular dysfunction remains a matter of discussion. ⋯ Severe left ventricular dysfunction is common in ICD pts. During long-term follow-up, the majority of these pts receive appropriate ICD shocks, which emphasizes the importance of SD prevention in this population. The frequent documentation of supraventricular arrhythmias (causing inappropriate ICD therapy) and nonfatal major arrhythmic events also reflects the presence of a worse arrhythmic substrate in this subgroup. Despite the poor initial prognosis associated with ventricular tachyarrhythmias in pts with severe left ventricular dysfunction, ICD therapy may contribute to a better long-term clinical outcome.

The last few years have seen significant progress in the treatment of advanced pulmonary arterial hypertension (PAH). The efficacy of new drugs has been proved mainly in idiopathic PAH or PAH associated with connective tissue diseases. As the pathophysiologic patterns are similar, it is reasonable to use these drugs also in Eisenmenger syndrome or in other congenital heart defects with PAH related to initial high pulmonary flow. ⋯ Though the number of patients was small, our initial experience with the new specific drugs for PAH in Eisenmenger syndrome showed promising results, with clinical and functional improvement and without adverse effects.

Hypertrophic cardiomyopathy is a genetic disease inherited as an autosomal dominant trait associated with risk of sudden death. The majority of cases of sudden death occur in young adults with no or few symptoms, which underlines the importance of risk stratification as a basis for selecting a therapeutic strategy. Implantable cardioverter-defibrillators are indicated in patients resuscitated following cardiac arrest, and those with sustained ventricular tachycardia or two or more risk factors identified in non-invasive tests. ⋯ In this group of patients, considered to be at high risk for sudden cardiac death, a considerable percentage had ventricular tachycardias that were correctly identified and treated by the implantable cardioverter-defibrillator. The percentage of patients with appropriate therapies was slightly higher in the group who had a cardioverter-defibrillator for secondary prevention of sudden death (aborted sudden death or sustained ventricular tachycardia). In patients with an implantable cardioverter-defibrillator for primary prevention, non-sustained ventricular tachycardia was the risk factor with the highest predictive value. An association of risk factors was also predictive of arrhythmic events.

Levosimendan in daily intensive care practice--the experience of 15 centers. Background, methods and organization of the PORTLAND study.
The LIDO and RUSSLAN trials showed that levosimendan was well tolerated and had a stronger hemodynamic effect than dobutamine and a positive impact on prognosis. There are, however, few data regarding its effectiveness and safety when used in an everyday clinical setting. ⋯ The therapy was judged safe in the absence of any serious adverse event with a probable or undetermined causal relationship with levosimendan. Primary endpoint evaluation: Patients reached the primary endpoint when levosimendan was both effective and safe according to the above definitions.

In the process of establishing a diagnosis, the clinician calculates the disease probabilities, which keep changing as data is gathered from clinical history, physical examination and laboratory and imaging data. This paper sets out to explain, in a simple and practical way, how the characteristics of a test can be applied in order to determine post-test probability, that is, the probability of a specific disease being present. ⋯ To calculate the post-test probabilities, the sensitivity and specificity of the test are used to calculate the positive and negative predictive values for a determined pretest probability, as well as likelihood ratios (through graphics or nomograms, approximations or equations). Among the test characteristics, the most useful for calculating post-test probabilities are likelihood ratios, which have several advantages over sensitivity and specificity.
