To assess the causes and pattern of hand injuries in patients with isolated unilateral acute hand injuries managed at the Kenyatta National Hospital (KNH). ⋯ Occupational hand injuries, a largely preventable problem, are the most prevalent cause of hand injury. This offers opportunity for strategies in preventing a large number of hand injuries by initiating safety measures at the work place.

Rachs-1 system in risk stratification for congenital heart disease surgery outcome.
The Risk Adjustment in Congenital Heart Surgery (RACHS-1) system has been used as a benchmark to compare surgical results in developed countries. Its ability to stratify postoperative mortality risk has been validated in several developed countries, however, this has not been examined in a developing country. ⋯ The RACHS-1 system did not adequately stratify risk in a low case load setting. The use of the RACHS-1 method as a benchmark to compare surgical results of paediatric cardiac surgery services in developing countries may be limited.

A 30-year-old farmer was bitten by his camel on the left side of the neck. He suffered immediate loss of consciousness but recovered three days later with right-sided hemiplegia and complete aphasia. ⋯ He was managed conservatively with physiotherapy and low dose aspirin. At six months of follow up, his speech remains slurred but comprehensible and is able to walk with a walking stick.

To highlight the pertinent management problems of bowel perforation following blunt abdominal trauma. ⋯ Peritonitis following a bowel perforation after blunt abdominal trauma is often present at the time of presentation and diagnosis is usually made. In the few doubtful cases, often in patients presenting soon after trauma, X-ray and trans-abdominal ultrasonography will assist in making a diagnosis. Delayed presentation still accounts for a high mortality in bowel perforation following blunt abdominal trauma.
