[A case report of congenitally bicuspid aortic valve and ascending aortic aneurysm treated by wheat operation].
A 59-year-old man who complained of palpitation was diagnosed as having a congenitally bicuspid aortic valve, severe aortic regurgitation, and an ascending aortic aneurysm. He underwent aortic valve replacement and conduit replacement by the modified Wheat technique. ⋯ Postoperative angiogram revealed no aneurysm formation of the aortic root or paravalvular leakage. This case suggest that aneurysm of the ascending aorta with aortic regurgitation is more effectively treated by the modified Wheat technique if cephalad displacement of the coronary ostium is not extensive.

[A case of postoperative chylothorax successfully treated by chemical pleurodesis with OK-432].
Recently, cases of postoperative chylothorax have increased concomitantly with the increase in number of aggressive thoracic surgery. Chylothorax was complicated on the 6 th postoperative day in a 69-year-old male who underwent right lower lobectomy with mediastinal lymph node dissection for adenocarcinoma of the lung. ⋯ By this procedure, the chylous fistulas were completely cured. Application of OK-432, even in such a small dose as 1.5 KE, appeared very useful for the treatment of postoperative chylothorax.

We developed a new method to monitor left atrial pressure in the patients undergoing open heart surgery. The advantages of our method are 1) freedom from complications relating its removal (eg. bleeding) because of its transvenous insertion, and 2) simultaneous monitoring of both right and left atrial pressures by a single catheter. ⋯ We used this catheter in 16 infants and children, and found that effective hemodynamic monitoring was obtained without any complications. We recommend this method for the postoperative management of open heart surgery in the pediatric patients.

A case of aneurysm of the mitral valve associated with aortic valve regurgitation is presented. The patient was 45-years-old female who complained of palpitation and underwent both aortic and mitral valve replacement. ⋯ The transesophageal echocardiography showed the aneurysm of the anterior mitral valve leaflet toward the left atrium clearly. This report describes that transesophageal echocardiography appears to be available for accurate diagnosis of the mitral valve disease, especially for the mitral valve aneurysm.

There are few reports of traumatic lung cyst in Japan. The 1st case was 20-year-old man with traumatic lung cyst from motor vehicle accident. ⋯ The 2nd case was 7-year-old girl with traumatic lung cyst caused by motor accident. The traumatic pneumatocele was not yet reduced in size four months after initial injury.
