Vaccines are a cornerstone in any pandemic influenza preparedness plan. Research and development of new influenza vaccines are essential for global preparedness. There are major differences between countries regarding production capacities, priorities and resources for pandemic influenza vaccine policy and planning. ⋯ The seasonal flu vaccine, as is in the current event, will probably not be effective against pandemic strains. Development efforts should be focused on developing broad-spectrum vaccine against both seasonal and pandemic viruses. Equitable distribution of scarce resources in the world requires attention to ethical dilemmas by a multinational and multidisciplinary team.

Chronic Obstructive Pulmonary Disease (COPD), a common disease worldwide, refers to two frequently coexisting lung diseases, chronic bronchitis and emphysema. Physiologically, COPD represents a disruption in ventilation and in the exchange of gases in the lungs. Laboratory tests indicate elevated CO2 level, gradual reduction of the levels of oxygen and pH in arterial blood and elevated PeCO2. It does not include other obstructive diseases such as asthma. ⋯ Most of the patients (92%) ventilated for acute respiratory failure due to chronic obstructive pulmonary disease (COPD) needed MV for only between 40-47 hours.

Medical child abuse or its better known name, Munchausen's Syndrome by Proxy, is a form of child maltreatment combining physical and emotional abuse as well as child neglect. As in other forms of child abuse and neglect that pediatricians encounter, the diagnosis is medical and is based on the existence of actual or potential harm to the child. ⋯ In this article we describe medical child abuse and the means of diagnosing the syndrome. It is also stressed that, as in the case of any other form of child maltreatment, reporting is mandatory.

Allen-Masters syndrome, first described at 1955, is considered an uncommon etiology for chronic pelvic pain. The syndrome associates traumatic delivery to lacerations found on the posterior leaf of the broad Ligament. The authors report the clinical and laparoscopic findings of a female patient who was admitted to their department for evaluation due to chronic pelvic pain. ⋯ In the laparoscopy the authors reported finding bilateral Lacerations in the posterior leaf of the broad Ligament. Coagulation of both lacerations was performed and the patient reported significant improvement in her complaints. The relevant literature was reviewed and the article discusses the process of diagnosis and treatment.

Burn injuries are very frequent and afflict approximately 1% of the population yearly. They are a source of heavy medical burden to medical systems worldwide. In the US alone, about 2 million burns are treated by medical staff yearly, and about 75,000 burns are serious enough to require hospitalization. ⋯ Fluid resuscitation is mainly managed using the Parkland equation. The treatment of the burned skin is by one of two regimes--the conservative regime (frequent redressing of the burn site, hygiene and antimicrobial treatment) and the surgical regime (early intervention with debridement, skin implantations etc.). Several different studies have shown a decrease in the mortality rate of severe burn patients who have undergone an early surgical regime in comparison to conservative treatment.
