The assessment of the role of transesophageal echocardiography and invasive tests with pulmonary modified catheter to monitor the preload indexes in patients in intensive-care-unit after cardiac coronary surgery. ⋯ Our study showed in patients soon after coronary cardiac surgery deltaVAo is the only predictor of "fluid responsiveness" and of ventricular compliance.

A 46 year old male smoker was admitted for severe continuing hemoptysis. Chest-X-rays and chest computed tomography revealed nodular infiltrates and bilateral hemothorax. Fiberoptic bronchoscopy resulted to non-diagnostic cytological and microbiological findings. ⋯ The patient received several courses of chemotherapy but he died one month later. The clinical, radiological, pathological, histochemical and therapeutic aspects of the disease are discussed. Angiosarcoma, a rare tumour with poor prognosis should be taken into consideration in the differential diagnosis of hemoptysis.

ReviewBi-level home ventilators for non invasive positive pressure ventilation.
With the widespread use of non-invasive positive pressure mechanical ventilation, great efforts have been made to produce machines, the bi-level home ventilators, which are less sophisticated, cheaper and able to better compensate air leaks with respect to the domiciliary volume-target machines and the traditional ventilators used in intensive care unit. As consequence of quick technologic evolution, bi-level home ventilators may be nowadays successfully applied for both the non-invasive ventilatory domiciliary treatment of chronic respiratory failure and the management of acute respiratory failure especially outside the intensive care setting. In this paper, the author describes the technical aspects, the individual characteristics and the clinical applications of the most common used bi-level ventilators.

Randomized Controlled Trial Clinical TrialInspiratory muscle workload due to dynamic intrinsic PEEP in stable COPD patients: effects of two different settings of non-invasive pressure-support ventilation.
In severe stable hypercapnic COPD patients the amount of pressure time product (PTP) spent to counterbalance their dynamic intrinsic positive end expiratory pressure (PEEPi,dyn) is high: no data are available on the best setting of non invasive pressure support ventilation (NPSV) to reduce the inspiratory muscle workload due to PEEPi,dyn. ⋯ In conclusion in severe COPD patients with chronic hypercapnia the inspiratory muscle workload due to PEEPidyn is high and is reduced by NPSV at a greater extent when ventilator setting is tailored to patient's mechanics.
