Randomized Controlled Trial Comparative Study Clinical Trial
Cotrel traction, exercises, casting in the treatment of idiopathic scoliosis. A pilot study and prospective randomized controlled clinical trial.
A pilot study of ten individuals with adolescent-onset idiopathic scoliosis demonstrated that a week of Cotrel traction and exercises did not improve curve correction obtained by the application of an elongation, derotation, flexion (EDF) cast. There was, however, a significant improvement on lateral bending correction during this period. A prospective, randomized, controlled clinical trial showed that the exercise programme and not the traction was responsible for rendering the spine less rigid.

Bone cement was used as an adjunct to internal fixation of 14 supracondylar femoral fractures. The fractures were stabilized with ASIF plates. In all these fractures severe osteoporosis made a stable fixation impossible without the use of bone cement. ⋯ No infections were seen. It is concluded that use of bone cement as an adjunct to the internal fixation of supracondylar fractures secures a stable fixation of even the most severe osteoporotic femora, and this aids healing in most cases of such fractures. The method is recommended in selected cases of elderly patients.

Fifteen cases of isolated fracture of the first rib are submitted. The mechanism of the fracture is discussed. It proved compatible with the descriptions in the literature, caused either by direct trauma to the shoulder, a sudden violent contraction of juxtacostal muscles; or else it was a chance finding, without any history of trauma and as such interpreted as a fatigue fracture. A case of Horner's syndrome complicating a fracture of the first rib is also described.

During the years 1950-1973, 229 cases of Achilles tendon rupture were diagnosed in the city of Malmö. More than half of the injuries were caused by sporting activities, the most common being badminton and football (soccer). ⋯ During the period investigated the incidence of Achilles tendon rupture increased in the city more than could be accounted for by changes in the population. This increase could only be partly explained by the increasing interest in sporting activities in the population at risk.

A case of peroneal nerve palsy due to superior dislocation of the proximal tibio-fibular joint is described. Emphasis is placed on the importance of early diagnosis and treatment.
