Case ReportsPneumomediastinum and subcutaneous emphysema following surgical extraction of mandibular third molars: three case reports.
Three cases of subcutaneous emphysema following surgical extraction of lower third molars are presented. In two of the cases, pneumomediastinum developed. The direct cause of these complications is the combination of the use of an air turbine dental handpiece and the flap design. The propagation of the emphysema and means for its prevention are discussed.

A study was carried out to compare the clinical features of 102 subjects suffering from burning mouth syndrome (BMS) and 43 age- and sex-matched control subjects. In comparison with those in the control group, the BMS subjects reported a significantly higher prevalence of dry mouth, thirst, taste and sleep disturbances, headaches, nonspecific health problems, pain complaints, and severe menopausal symptoms, but no significant differences in other oral or dental features or in the prevalence of candidiasis infection. In addition, there was no hematologic evidence of a nutritional disturbance in more than 90% of the BMS subjects tested, but immunologic abnormalities and an elevation of the erythrocyte sedimentation rate were found in more than 58% and 63% of the BMS subjects, respectively. These findings demonstrate those features which distinguish BMS subjects from age- and sex-matched control subjects but provide no confirmatory evidence for many of the etiologic factors frequently suggested for BMS.

Case ReportsCervical mucocele (plunging ranula): an unusual case of mucous extravasation cyst.
An unusual case of a mucous extravasation cyst that manifest itself solely as a cervical swelling is reported. As a result of this unusual symptom, diagnosis and appropriate treatment were delayed, and the condition finally resolved after the ipsilateral sublingual salivary gland was excised and the cervical fluid was aspirated. Cervical mucoceles have also been reported in dogs, and the veterinary literature pertaining to this is briefly reviewed.

A set of descriptors for the diagnosis of dental pain syndromes.
Two hundred patients with pain in the head and neck area completed the McGill-Melzack Pain Questionnaire before a diagnosis was made or treatment was recommended for their complaints. The class and subclass of the chosen pain descriptors were compiled for each of three broad categories of pain: pain of periodontal origin, pain of pulpal origin, and pain associated with temporomandibular syndrome (TMS). ⋯ A larger sample and further study are indicated to determine whether unique word patterns may be developed for individual oral pain syndromes. Ease of administration and the capability of detecting consistent patterns of pain descriptors make the McGill-Melzack Pain Questionnaire a potentially useful tool for the diagnosis and management of pain syndromes in dentistry.

Rheumatoid disease and related arthropathies. I. Systemic findings, medical therapy, and peripheral joint surgery.
Rheumatoid arthritis and related arthropathies may produce a wide, confusing range of problems affecting the temporomandibular joint and ultimately the lower face. In order to understand the evolution of therapy for TMJ and facial problems, a general update of overall disease characteristics, current medical therapy, and peripheral joint surgery is presented. This background is integrated into a rationale for treatment of rheumatoid problems affecting the mandible.
