Comparison of percutaneous intramedullary Kirschner wire and interfragmentary screw fixation of displaced extra-articular metacarpal fractures.
the management of the displaced extra-articular metacarpal fractures is still a subject of debate in the literature. The purposes of this study were to report the outcomes of unstable extra-articular metacarpal fractures treated by using intramedullary Kirschner wires or inter-fragmentary screws and to determinate which techniques provide better clinical and radiographic results. ⋯ our results indicate that both procedures are effective in the treatment of displaced extra-articular metacarpal fractures. However, we believe K-wires represent the gold standard of treatment for displaced fractures of the metacarpal neck. Instead, screws are more effective for spiroid shaft fractures, while displaced fractures of the base may be treated with either screws or wires.

2-Chloroprocaine is a local anesthetic with a very short half-life and a favorable evolution of spinal block for ultra-short outpatient procedures. The aim of this retrospective study is to evaluate the clinical impact of the introduction of spinal 1% 2-chloroprocaine compared to general anesthesia at the ARS Medica Clinic (Switzerland). ⋯ The right selection of the local anesthetic makes spinal anesthesia a suitable anesthetic technique for ultra-short outpatient procedures. If short acting local anesthetics are involved, spinal anesthesia could be competitive versus general anesthesia.

Effectiveness of Back School program versus hydrotherapy in elderly patients with chronic non-specific low back pain: a randomized clinical trial.
Chronic low back pain (CLBP) is a major cause of disability, for which clinical practice guidelines suggest exercise programs, such as Back School program (stretching and selective muscle reinforcement techniques) and Hydrotherapy technique, as an effective treatment to reduce pain intensity and disability. ⋯ Back School program and Hydrotherapy could be valid treatment options in the rehabilitation of non-specific CLBP in elderly people. Both therapies proved to be effective and can be used in association with other rehabilitation programs. We believe that Back School program should be favored for its simplicity and the small number of resources required.

Lung ultrasound (LUS) is becoming more and more utilised in the clinical field inadults, children and neonates in course of respiratory diseases. It can be done at bed side and repeated as muchis needed without risks (namely irradiation). ⋯ The use of LUS in neonates may indicatein real time the diagnosis of the respiratory disease. Should be considered as an extension of the clinicalexam and must be done by the clinician in charge of the patient.

Propose further evidence for reliability and validity of Italian translation and adaptation of Fear Avoidance Beliefs Questionnaire, (FABQ) originally built-up by Waddel et al. (1993) METHODS: 250 participants, inpatient and outpatient, suffering from acute or chronic back-pain have been consecutively recruited from various Italian physiotherapy and functional rehabilitation centres. All participants were administered FABQ and QUALEFFO, to evaluate if avoiding behaviors can provoke disability independently from experimented pain. ⋯ Pain-related fear and fear-avoidance beliefs are specific index which have to be considered in the first assessment phases, to prevent their effect on global functioning and on patients' quality of life. FABQ pointed out good reliability and validity propriety in Italian version. FABQ seems to be a brilliant instrument for multidisciplinary clinical practice in pain problem approach.
