Paravertebral block (PVB) has been proposed as an alternative to General anaesthesia (GA) for breast surgery. It provides good operative anaesthesia, good pain control with little adverse effects. Six women older than 80 year were selected. ⋯ All patients declared absence of pain and nausea and that they were satisfied with the procedure. The use of PVB allows elderly patients to undergo ambulatory surgery for the treatment of breast cancer with satisfaction. This technique allows a short recovery and adequate postoperative pain relief with reduced hospital costs.

The treatment of isolated fractures of the greater tuberosity of the proximal humerus is still debated, especially in the case of minimally dispaced fractures. Differently from other proximal humerus fractures this kind of injury frequently affects young patients with high functional request. The aim of this study is to assess the outcome of patients treated conservatively for greater tubero-sity isolated fractures, comparing the clinical results in patients with different direction of fracture displacement. ⋯ Data suggest that postero-superior displacement leads to poorer outcomes than undisplaced or displaced in other directions fractures treated conservatively.

We compared the following stretchers to identify the better and safer to immobilize a fallen rider on the track. The stretchers were: Northwall's Innovation Skidboard, Ferno's Scoop EXL65 and anonymous aluminium scoop stretcher. We performed tests on track with several repetitions on a 25 meters path including asphalt, curb, natural and synthetic grass, and gravel. ⋯ Skidboard is faster than other stretchers to complete the fallen rider's rescue on the track. It is the only spineboard that allows to tie the patient to the stretchers from head to toe, which does not happen with the aluminum scoop and only partially with the Exl65. Skidboard is better than others to rescue fallen and traumatized riders on the tracks quickly and safely.

Late interventricular septal defect after blunt chest trauma: a case report and a review of the licterature.
Cardiac injuries from blunt chest trauma are usually the result of motor vehicle accidents; cardiac injuries result in 20% of such accidents, but they are also associate to mild-to-moderate sports injuries. The severity of external injuries does not necessary reflect the severity of cardiac trauma. ⋯ We report a case of traumatic rupture of the interventricular septum caused by a blunt chest trauma following a car crash and its successful surgical repair. In this particular case the clinical signs of interventricular shunt began evident 12-14 days after the accident.

Spinal anaesthesia is an easy and reliable technique. Factors limiting its use in the ambulatory setting include delayed ambulation, risk of urinary retention and pain after block regression. ⋯ Moreover, the preservative free 2-chloroprocaine solution showed a very low risk of urinary retention and transient neurological symptoms when compared with bupivacaine and lidocaine. The aim of this article is to evaluate if the neuraxial administration of short-acting local anaesthetics renders spinal anaesthesia a suitable technique for ultra-short surgical procedures.
