The authors report the case of a 31 year-old male with right atrial myxoma whose clinical manifestations included: malaise, dizziness and dyspnea on great exertion. At physical examination, a tricuspid systolic and diastolic murmur of mean intensity was found. ⋯ The patient underwent surgical removal of the tumoral mass and diagnostic confirmation was made with the histopathologic study of the 75 x 50 x 40 mm piece. The patient obtained clinical, electrocardiographic and radiologic remission.

A 34 years old woman, without previous cardiac symptoms, suffered a closed thoracic trauma in a car accident. Three days after presented dyspnea and orthopnea. Physical examination revealed a systolic murmur consistent with ventricular septal defect (VSD). ⋯ Surgical intervention was indicated and VSD closure was performed with a patch, through a right atrium approach. Third degree atrioventricular block developed after surgery and a definitive pacemaker was implanted. The patient is asymptomatic and without murmurs in postoperative follow-up.

Case Reports[Fracture and intracoronary retention of a guidewire catheter in percutaneous transluminal angioplasty. A case report].
We present a rare coronary complication of angioplasty in which the steerable catheter was broken within the circumflex artery. Surgical removal of the fragment was possible and the patient had an uneventful recovery. Considerations on this serious problem, its possible causes, prevention and management are made.

Comparative Study Clinical Trial Controlled Clinical Trial[Comparative study of the use of nifedipine and captopril in hypertensive emergencies].
Forty patients with hypertensive crises, whose pressure levels had not satisfactorily decreased after intravenous furosemide 20 mg, were selected in the Emergency Ward. These patients were divided in two groups. The ones in group A received sublingual nifedipine 10 mg and the others in group B received oral captopril 50 mg. ⋯ No side effects were observed. All patients were discharged from the emergency unit and were sent for follow-up in the outpatient service. The use of nifedipine and captopril has proved to be an effective treatment for hypertensive crises.
