[Patient's delay and doctor's delay in the primary treatment cases of pulmonary tuberculosis detected by subjective symptoms].
Delays in case finding were studied for the primary tuberculosis patients who were discharged from our hospital in 1987. Of 321 tuberculosis patients who were released from our hospital in 1987, 171 patients received primary treatment. Eighty one of them had been detected by their symptomatic visits. ⋯ Total delay until definite diagnosis by 50% and 80% of all diagnosed were 1.6 months, and 3.5 months respectively. Patients with total delay of more than three months were younger in age and were hospitalized longer than patients with total delay of less than 3 months. All cases with total delay of more than three months were smear positive.

Case Reports[A case of miliary tuberculosis with prolonged high fever for more than 2 months under antituberculous therapy].
A 28 year-old male was admitted to our hospital with persistent cough and high fever. He was diagnosed to have miliary tuberculosis by the transbronchial lung biopsy specimen and tuberculous choroidal lesions in the ocular fundus. ⋯ In spite of the fact that the bacilli were sensitive to the antituberculosis drugs used and he had no other complications, high fever persisted and lasted for more than 2 months. When tuberculosis is suspected, and antituberculosis treatment is tried to observe its clinical response, the presence of similar cases mentioned above should be taken into consideration.

[Diagnostic value of bronchoscopy in diagnosis of pulmonary tuberculosis: bronchial aspirate, bronchial washing and transbronchial lung biopsy].
The diagnosis of pulmonary tuberculosis is confirmed by the detection of Mycobacterium tuberculosis in sputum. Bronchoscopy has been used for diagnosis of various pulmonary diseases. ⋯ Transbronchial lung biopsy showed positive findings of tuberculosis in 75.8% out of 33 specimens. 3) Out of 88 sputa taken before bronchoscopy and 50 sputa after bronchoscopy, smear positivity was 0% and 12%, and culture positivity was 54.5% and 40% respectively. Gastric lavage culture positivity was 29.4% in 17 cases examined. 4) Diagnosis of tuberculosis was made rapidly in 28 cases (30.4%) by smear positive results of bronchial aspirate, bronchial washing and sputa after bronchoscopy, and relatively rapidly in 20 cases (21.7%) by transbronchial lung biopsy.

Case Reports[A case of empyema after plastic ball plombage cured by air-plombage method].
Conventionally, thoracoplasty has been conducted for empyema space after removal of plastic ball for empyema cases after plastic ball plombage. We applied air-plombage method for empyema as a new operative technique. The patient was a 56-year-old man who had had 37 plastic balls implanted for the treatment of pulmonary tuberculosis 39 years ago. ⋯ We selected air-plombage method rather than thoracoplasty as postoperative worsening of pulmonary functions was anticipated by the latter. Postoperative improvement of pulmonary functions could be explained by reinflation of the residual lobe due to removal of plastic balls and the capsule. When conventional thoracoplasty is carried out to obtain satisfying closure of the empyema cavity, pulmonary functions are always deteriorated, while air-plombage method is followed by slight improvement of pulmonary functions.(ABSTRACT TRUNCATED AT 250 WORDS)
