Randomized Controlled Trial Clinical Trial[Pre- and intra-operative administration of epidural morphine provides good postoperative pain relief after laminectomy].
To evaluate the postoperative analgesic effect of epidural morphine administered at different timing in lumbar spine surgery. ⋯ Preoperative or intraoperative administration of epidural morphine could provide satisfactory analgesia in lumbar spine surgery during the first 24 h postoperatively.

Case ReportsPropofol anesthesia in a patient with myasthenia gravis--a case report.
Experience with the use of propofol for induction and maintenance of anesthesia in patients with myasthenia gravis is limited in the literature. We report, in this case, the well prepared and successful use of continuous propofol infusion without conventional neuromuscular blocking agents in a patient with Ossermann stage IIb of myasthenia gravis undergoing laparoscopic cholecystectomy.

Case ReportsRecurrent epistaxis following nasotracheal intubation--a case report.
Epistaxis is one of the common complications of nasotracheal intubation. Clinical patterns of all nasal bleeding are mild and may stop spontaneously in most patients. Serious nasal bleeding requiring hospitalization are rare. ⋯ Although numerous publications have reported the cases of initial epistaxis caused by nasotracheal intubation, recurrent epistaxis was reported rarely. We experienced a case of recurrent epistaxis occurring in the 6th, 8th, 15th and 18th day after nasotracheal intubation. Herein, we describe the clinical events and discuss the causes of epistaxis.

[The incidence of double-lumen tubes displacement after positioning of patients during anesthesia].
Although many reports have described that unexpected displacement of double-lumen endobronchial tube (DLT) may be caused by patient positioning, however, in Taiwan the matter has not been reported, nor its incidence has been determined. The purpose of this study was to determine the incidence of DLT displacement after patient positioning. ⋯ Our result suggests that there is a high incidence of DLT displacement during patient positioning. Whenever the patient is repositioned, we recommend that constant vigilance is necessary to guard against tube displacement. Attention should be focused on possible causative factors so that the incidence can be decreased and the safety of DLT application assured.

Randomized Controlled Trial Clinical TrialInterpleural bupivacaine for pain relief after transthoracic endoscopic sympathectomy for primary hyperhidrosis.
Interpleural local anesthetic has been reported to provide good postoperative pain relief in patients receiving thoracotomy or upper abdominal surgery. However, there were few reports regarding interpleural local anesthetic for postoperative pain relief in patients receiving transthoracic endoscopic sympathectomy for palmar hyperhidrosis. The aim of the present study was to evaluate the effect of interpleural bupivacaine for postoperative pain relief in patients receiving transthoracic endoscopic sympathectomy for palmar hyperhidrosis. ⋯ Interpleural bupivacaine significantly decreased the intensity of postoperative pain and morphine consumption in patients undergoing transthoracic endoscopic sympathectomy for palmar hyperhidrosis. We are looking forward to seeing that with the addition of epinephrine to the anesthetic solution and a considerable increase of dosage and/or volume of bupivacaine may provide a better and longer pain relief, about which further studies are needed.
