The population is well aware today that the emergency doctor holds a strong position in the field of preclinical care. The time of onset of expert care of the emergency patient is of great significance in regard to survival and prevention of further damage. ⋯ In respect to content and form of training a proposal has been made representing a minimal requirement. It should be assured in modern day ambulance care that only qualified physicians participate who are familiar with the special situations encountered in emergency medicine.

Comparative Study[Therapy of orthostatic dysregulation. Comparative studies on anti-hypotensive sympathomimetics with a tilt-table test].
In 7 patients suffering from orthostatic hypotension the early behaviour of arterial mean blood pressure was studied in the tilting table experiment under influence of etilefrine, norfenefrine, gepefrine (D-(+)-1-(3-hydroxyphenyl)-2-aminopropan-(2R, 3R)-hydrogentartrat) and of its I-isomere. Significant improvement of early orthostatic disregulation of the arterial pressure could be only observed under gepefrine. Amelioration seems to be induced by sensitization of the pressoreceptors.

It is reported about 73 traumatic ruptures of the spleen in a period of 19 years. The most frequent cause for rupturing were traffic accidents. The most important diagnostic procedure was peritoneal lavage, a rapid and well-aimed method. ⋯ Prognosis depends on the time from accident to operation as well as on the severity of accompanying injuries. The course and cause of lethal courses are summarized. Ten patients who died in the postoperative phase, had polytraumatic injuries with three of more injured organs.

[The uric acid lowering effect of protein-rich diets. Behavior of human uric acid metabolism under reducing diet forms with varied protein content].
The effect of a 60 g protein diet was compared to a 120 g protein diet. Under nutrition with 60 g protein/day the uric acid clearance dropped from 6.3 +/- 2.2 ml/min to 4.9 +/- 1.6 ml/min. ⋯ Therefore serum uric acid concentration dropped from 7.7 +/- 0.5 mg/dl to 6.4 +/- 0.5 mg/dl. The supply of 120 g protein/day has such a strong uricosuric action that the otherwise occurring inhibition of uric acid clearance during weight reduction can be balanced.

[In-vitro efficacy of topical antibiotics on bacteria from burns].
The influence of topical antimicrobial agents on bacterial growth was determined with an agar diffusion technique. 349 isolates were tested with mafenide acetate and silver sulfadiazine, 226 isolates were tested with povidone iodine. None of these agents was likewise effective against all organisms of the same bacterial strain. ⋯ Both agents influenced the growth of pseudomonas (40 to 65%). The results obtained with povidone iodine led to the consideration that the agar diffusion method might not be suitable for testing this drug.
