To obtain data regarding admissions of U19 rugby players to spinal injury units in Great Britain and Ireland and to compare this with a recent peak in presentation in Scotland. To assess the current state of data collection and subsequent analysis of serious neck injuries. To analyse the mechanism of injury in this group of at-risk players. ⋯ U19 rugby players continue to sustain serious neck injuries necessitating admission to spinal injury units with a low but persistent frequency. The recent rate of admission in Scotland is disproportionately high when the respective estimated playing populations are considered. While more injuries were sustained in the tackle, spinal cord injury was significantly more common in neck injury sustained in the scrum (p<0.001). No register of catastrophic neck injuries exists despite repeated calls over the past three decades, and a study such as this has not been reported before. Data collection of this serious category of injury is incomplete and very variable across the home unions, as a consequence a large proportion of the serious neck injuries that have occurred in U19 players over the past 14 years have not been analysed. Rigorous data collection and analysis have to be established so that problem areas of the game such as scrum engagement and the tackle can be made safer.

Respiratory diseases such as asthma, chronic cough, recurrent respiratory infections and various upper airways conditions are common in elite athletes, but these conditions are often underdiagnosed and undertreated. Recurrent cough, often observed after exercise, is the most commonly reported symptom in athletes, particularly winter athletes, but it does not predict airway function; its intensity correlates with the dryness of inspired air but may not be associated with airway hyper-responsiveness. ⋯ These conditions should be recognised and treated properly according to current guidelines, although how these last apply in the athlete is uncertain. Furthermore, regulatory agencies' restrictions on the type of drugs allowed for therapeutic use of these conditions in competitive athletes should be checked.

Leisure-time physical activity (LTPA) has been associated with better mental and physical health particularly in cross-sectional studies. Purpose To longitudinally assess the association between LTPA, changes in LTPA and health-related quality of life (HRQL). ⋯ These findings provide support for an association between LTPA, long-term changes in LTPA and several aspects of HRQL (especially in relation to mental domains) extending previous cross-sectional findings.

To evaluate whether deficits of elbow flexor and extensor muscle strength exist in lateral epicondylalgia (LE) in comparison with a healthy control population. ⋯ Small significant deficits of elbow extensor and flexor muscle strength exist in the affected arm of unilateral LE in comparison with healthy controls. Notably, comparing elbow strength between the affected and unaffected sides in unilateral epicondylalgia is likely to underestimate these deficits. Concussion head injuries are common in high school athletes. The Sport Concussion Assessment Tool-2 (SCAT2) has been recommended and widely adopted as a standardised method of evaluating an injured athlete with a suspected concussion. Sideline return to play decisions can hinge on the results of a SCAT2 score. However, most athletes will not have had baseline testing performed for comparison if injury occurs. Therefore, establishing of age-, sex- and sport-matched normative data for the high school athlete population is critical. ⋯ Non-concussed high school athletes scored near the total possible in most domains of the SCAT2 with the exception of concentration testing and balance testing. All athletes were able to complete the double-leg stance at baseline; however, there was significant variability of tandem and single-leg stance. Baseline testing is important when considering balance tests. Concentration testing in high school athletes is unreliable because of high baseline error and is likely to result in a high rate of false positives and false negatives. Return to play decisions should not rely on concentration testing without a baseline test for comparison.
