The realisation and reliability of the halothane-caffeine contracture tests in children to detect the susceptibility to malignant hyperthermia (MH) is still controversial. The present study concerned 26 children aged 2 to 13 years, (mean 9.5 +/- 1.3 years) who were tested either because of a personal symptomatology (14 cases) or as a member of a susceptible MH family (12 cases). ⋯ Furthermore comparison of threshold concentrations of halothane and caffeine as well as the 32 nmol caffeine-induced contractures dit not show significant differences related to age. These results support the possibility to perform under good conditions and with good reliability the diagnostic test of susceptibility to malignant hyperthermia in children from 2 years on.

The authors report the case of a 33 month-old child who presented some of the classical symptoms of the Henoch-Schoenlein purpura - arthritis, purpuric lesions of the lower extremities - associated with the full-blown picture of a post-infectious glomerulonephritis - low level of C3, proliferation of mesangial cells, exudation with large number of leukocytes, and C3 glomerular deposits. These findings and others previously described suggest that the Henoch-Schoenlein purpura is a syndrome and that some of its manifestations may occur in patients with post-infectious glomerulonephritis.

Toxic shock syndrome (TSS) is reported in 2 children. TSS occurred 10 days after an accidental injury of the fore foot in one case and 5 days following surgery for severe uretero-vesical reflux in the other. ⋯ Toxin-induced mediators such as interleukin I and Tumor Necrosis Factor have been incriminated in septic shock with multivisceral involvement. As in our 2 cases, the syndrome may be delayed or even absent.

[Fetal supraventricular tachycardias and their treatment. Apropos of 23 cases].
The management of fetal heterotopic tachycardias is reviewed from a cooperative study involving 23 cases treated by French Pediatric Cardiology Centers at Angers, Brest, Nantes, Rennes and Tours. There were 17 cases of supraventricular tachycardia (SVT), 5 of flutter and 1 of atrial tachycardia. Seventeen mothers were given an antiarrythmic treatment. ⋯ There was a good in utero response in 12 cases. Among the 23 patients, 5 died, all with hydrops fetalis at the time of diagnosis of SVT. Finally 2 newborns were successfully treated at the time of delivery by a direct puncture of the umbilical cord.
