Review[Treatment of peritoneal carcinomatosis with cytoreductive surgery and hyperthermic intraperitoneal chemotherapy].
Pinto Peritoneal carcinomatosis (PC) had for long been regarded as a terminal disease, characterized by a very poor survival and worthy of being treated with palliative therapy only. Cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) provide a promising additional treatment option for patients with peritoneal carcinomatosis, resulting in recently published series enable to obtain long-term survival. In spite of the need for more high quality studies, there is now a consensus among many international experts about the use of this new strategy as gold standard for treating with intent of cure selected patients with PC. We summarized the present status and possible future progress of this treatment modality, in particular outlining its rationale, current practice and general outcomes.

Case ReportsBleeding control by radiofrequency in penetrating trauma of the liver.
Termocoagulation by radiofrequency (RF) is widely used for ablation of liver neoplasms. Recently, innovative uses of RF were proposed, as to assist liver resection, to help partial splenectomy, or to treat ruptured hepatocellular carcinoma. A 21 years old man was admitted for massive haemorrhage from a right thoraco-abdominal knife wound. ⋯ During operation, considering the difficult exposure of the source of bleeding, it was taught to stop haemorrhage using RF termocoagulation, under ultrasonographic guidance. Termocoagulation of the pedicle of the liver segment VIII was performed. In this patient with haemorrhagic shock the RF method for bleeding control was very easy and effective, and avoided risks of morbidity due to a major procedure.

[Results of open repair of infrarenal abdominal aortic aneurisms in patients unfit for endovascular treatment].
Purpose of the study was to assess how the introduction of endovascular treatment has affected mortality and morbidity of the traditional surgery for infrarenal abdominal aortic aneurysms (AAA). ⋯ e 30-day mortality was 4% respectively in group A and 5.5% in group B (P = not significative, n.s.). The incidence of renal and ischemic peripheral complications was, respectively, 2% and 4% in group A, and 4.1% and 8.3% in group B showing statistical significance (P <.001). There were no documented statistically significant differences between the two groups in terms of cardiac and respiratory complications (P = n.s.). CONCLUSIONS. The results of the traditional surgery for the infrarenal AAA not suitables for endovascular repair suffer from the difficult anatomy of aorto-iliac district. Although the incidence of complications of open surgery is increased, the mortality is similar to anatomical not complicated aneurysms.

[The abdominal compartment syndrome and the importance of decompressive re-laparotomy].
the Abdominal Compartment Syndrome (ACS) is an increasingly recognized complication of both medical and surgical patients. The World Society of the Abdominal Compartmental Syndrome defined Intra Abdominal Hypertension (IAH) as a mean Intra Abdominal Pressure (IAP) ≥ 12 mm Hg and the ACS as IAP ≥ 20 mmHg (with or without an abdominal perfusion pressure < 60 mm Hg) that is associated with dysfunction or failure of one or more organ systems that was not previously present. The IAH contributes to organ failure in patients with abdominal trauma and sepsis and leads to the development of ACS. ⋯ in according to our experience and the results of the literature, we believe essential monitoring abdominal pressure in patients with abdominal laparotomy. The abdominal decompressive re-laparotomy is a useful procedure to reduce symptoms and improve the health of the patient.

The association between thyroid and parathyroid diseases is well known in the context of MEN, but we find also in nonsyndromic scenarios. In our study, were considered the latter. Between 2003 and 2008 in our Division of General Surgery 61 patients underwent operation for hyperparathyroidism, 13 for primary, 41 secondary and 7 tertiary. ⋯ The thyroid disease associated with tertiary IPT was in all of our cases benign. The association of thyroid and parathyroid disease is therefore relatively common. It follows therefore the importance of careful preoperative evaluation in patients with hyperparathyroidism, in order to avoid a reoperation on the neck, because of higher complication rate.
