Case Reports[Unusual cause of acute abdomen in adult patient: gallstone ileus].
Gallstone ileus is a rare complication of cholecystolithiasis. It causes 1-3% of the mechanical obstructions of the small bowel. It often affects patients between 63 and 85 years old. Pre-operative diagnosis is usually delayed 1-10 days because there is not a specific symptomatology. ⋯ Patient's performance status influences surgical strategy. In our experience, the patient underwent one-stage surgery because he was considered at low risk. Instead staged procedure with enterolithotomy and delayed cholecystectomy and fistula repair, is performed only in patients at high risk.

Tissue engineering technologies: just a quick note about transplantation of bioengineered donor trachea and augmentation cystoplasty by de novo engineered bladder tissue. C. Alberti Tissue engineering is a multidisciplinary scientific field that aims at manufacturing in vitro biological substitutes to enhance or replace failing human organs. ⋯ Nevertheless, the use of adult organ-specific cells shows many limitations, such as difficulties in their harvesting (potential complications associated with invasive biopsies) and their low proliferative ability. Therefore, various populations of either embryonic or adult stem cells and progenitor cells have been studied as useful cell sources for the tissue engineering. Bioreactors are essential in such technologies, both providing chemo-physical cell culture dynamic conditions, that mimic the in vivo environment, and allowing the assessment of responses of biological substitutes to different biochemical signals and mechanical forces.

[Etiology, pathophysiology and management of post-traumatic pneumomediastinum: our experience].
The Authors reported the cases of posttraumatic pneumomediastinum came to their observation over the past 8 years. The etiology, pathophysiology, diagnosis, treatment and blunt injures eventually associated are discussed. Conclude that in the absence of associated injuries the treatment and the course of post-traumatic pneumomediastinum are the same that spontaneous pneumomediastinum.

Comparative Study[Overnight surgery in proctology and anesthesia type: our experience on 320 patients].
Several evidences demonstrated that general anaesthetics associated with local anaesthesia show more advantages in comparison to spinal anaesthesia in proctological surgery. Aim of this study was to verify the efficacy of deep general anaesthesia associated with a loco-regional anesthesiological technique, in a grade population of 320 patients affected by haemorrhoidal disease and anal fissure. ⋯ Loco-regional anesthesiological technique with posterior block, associated to general anaesthesia, has been demonstrated efficacious for the treatment of the most frequent proctological pathologies. This method allowed a low incidence of early and late complications and a more fast resolution of clinical sequelae of the surgery.

Comparative Study[Incisional hernia prosthetic surgery: a prospective study comparing laparoscopic and open techniques].
Incisional hernia is a common complication of abdominal surgery and it is often a source of long-term morbidity. Surgical treatment include many different techniques and up to today the choice may be difficult. The employment of prosthetic materials has contributed to a remarkable improvement in the results of this surgery. We performed a prospective study to compare the outcomes after laparoscopic and open incisional hernia repair. ⋯ In our experience laparoscopic incisional hernia repair appear to be a safe, feasible and effective alternative to Rives-Stoppa technique.
