In 7 years between 1982-88, 122 complaints were lodged against the Accident and Emergency department of King's College Hospital. A high percentage mentioned more than one aspect per complaint. Commonest were those regarding attitude (37.7%), missed diagnosis (36.6%), waiting time (32.8%), cursory examination (14.7%) and poor communication (11.5%). ⋯ A high index of suspicion for the unusual and careful examination of patients would reduce complaints of missed diagnosis. Sufficient medical and nursing staff would reduce waiting time and improvements in communication with patients would keep this aspect to a minimum. Complaint investigation can be time consuming, when dissatisfaction is expressed explanations at the time of presentation by senior staff members may head-off a formal complaint.

As a result of the lorry explosion on the Fengate Industrial Estate, Peterborough on 22nd March 1989, 82 casualties arrived at Peterborough District Hospital. Despite the fact that casualties arrived at the A&E Department in large numbers before the Major Accident Plan could be implemented, all of the casualties had been assessed and either admitted or sent home within 3 h of the explosion. In the process several valuable lessons were learned, and their implications are discussed in this paper.

We report on the outcome of major trauma patients brought to the Kent and Canterbury Hospital in 1989 and compare it with 1987. There is an improvement which followed changes in the management of serious injuries.

Thirty-eight teaching hospital affiliated accident and emergency departments were surveyed by post to try to find if there had been any change in undergraduate medical student teaching over the past 10 years. Twenty-six departments replied. The results showed that although there has been an improvement in the teaching of A&E medicine to undergraduates in the past few years there are still some medical schools where an A&E attachment is not mandatory. In those departments providing teaching, there is wide variation in course duration and content.
