Case Reports[Coccidioidomycosis--differential diagnosis of lung infiltrates with peripheral eosinophilia].
A 20-year-old man developed pulmonary coccidioidomycosis after travelling in Mexico and California. Cardinal clinical symptoms were fever, pulmonary infiltrate with ipsilateral hilar adenopathy on the chest X-ray, and a 14% eosinophilia in the peripheral blood. ⋯ Coccidioidomycosis was diagnosed by serology, and Coccidioides immitis grew in the sputum culture. With ever more travellers to other parts of the globe coccidioidomycosis must be considered in the differential diagnosis of pulmonary infiltrates with peripheral eosinophilia.

[Serum neopterin determination for the additional safeguarding of blood transfusions. Our experiences with 76,587 blood donors].
Since October 1986, all volunteer blood donors in the Tirol of Austria have been tested for neopterin. Serum neopterin levels were raised in 1242 donors (1.6%), 650 of whom (52.3%) consented to another test four weeks after the donation. ⋯ Since the beginning of 1988, all donated blood with increased serum neopterin levels also had an IgM test for cytomegalovirus (CMV): nine of 243 samples tested (3.7%) indicated an acute CMV infection. The neopterin assay thus detects a variety of potentially harmful diseases or conditions which would not be revealed by the usually employed battery of routine tests.

Several days after not eating anything while drinking heavily a 45-year-old alcoholic developed severe ketoacidosis. He was admitted to hospital in a stable cardiovascular condition, but already in a comatose state with Kussmaul breathing. ⋯ There was a marked hypoglycaemia (40 mg/dl). Despite intensive treatment it was not possible to prevent multiple organ failure, of which the patient soon died.
