[COVID 19 - Hospital Admission in the First and Second Wave in Germany].
We analyzed patients' characteristics and hospital admission in Germany's first and second COVID 19 wave. ⋯ Patients' characteristics did not vary in Germany's first and second COVID 19 wave, but anticoagulation and dexamethasone were applied more frequently in the second wave. In addition, there were fewer thromboembolic complications in the second wave.

Case Reports[Cefiderocol in AML patient with neutropenic infection due to 4MRGN Klebsiella pneumoniae (VIM-1)].
The 68-year-old patient presented with fever, general malaise and physical weakness in neutropenia during a known relapse of acute myeloid leukaemia after allogeneic stem cell transplantation. ⋯ 4MRGN pathogens are Gram-negative rod-shaped bacteria that are resistant against four main bactericidal antibiotic groups (Acylureidopenicillins, 3rd generation cephalosporins, carbapenems, and fluoroquinolones) and thus difficult to treat. The present case demonstrates good clinical efficacy for cefiderocol even in pathogens resistant to antibiotics such as colistin and ceftazidime/avibactam and highlights the importance of antibiogram-tailored therapy.

Barrett's esophagus has been an important issue in clinical medicine for many years. In the 70 s of the last century, the association between gastroesophageal reflux disease and BE was detected. Only ten years later, the association between BE and adenocarcinoma of the esophagus was reported. ⋯ Only in case of malignant transformation of BE which may range from low-grade dysplasia to high-grade dysplasia and early Barrett's cancer, endoscopic therapy is indicated. At present, there are emerging techniques of artificial intelligence. Due to these rapid developments in BE management, it is important to keep an eye on the current status of BE management.

Comment[Neglected differentiations: Of relative and absolute risk reduction and of seat belts and vaccines].
It is often reported that established COVID-19 vaccines have an efficacy of 70 % or more in preventing serious illnesses or deaths. On the other hand, critics claim that the vaccines have actually shown less than 1 % efficacy in studies and are therefore of little use. Which statement is correct? And where do these discrepancies come from?

Vertigo has many different causal disorders, ranging from general dizziness and orthostatic regulation disorders to attacks of rotary vertigo. A targeted anamnesis and clinical examination can be used to narrow down the differential diagnosis. Questions about the type of dizziness, the duration and accompanying symptoms must be clarified. ⋯ But diagnostic imaging is also important. MRI can be used to confirm or rule out vascular causes (cerebral infarction or minor bleeding) and inflammatory lesions. Because the most serious misdiagnosis of dizziness is overlooking a stroke.
