[Local sorption therapy and its effectiveness in correction of endogenous intoxication in patients with acute purulent pyelonephritis and paranephritis].
The 118 operated patients with different forms of acute purulent pyelonephritis, including complicated with paranephritis were followed-up. Control group consisted of 65 similar patients who underwent traditional comprehensive treatment. Study group (n = 53) underwent additional wound dialysis with carbonic drainage-sorbent. ⋯ The use of sorption therapy in patients with purulent and destructive forms of pyelonephritis and paranephritis allowed to reduce postoperative complications in patients of study group by 11%, as confirmed by the positive dynamics of the proinflammatory and anti-inflammatory cytokines. Moreover, the effectiveness of wound dialysis is justified by accelerated rehabilitation of patients and the possibility of discharge for 3 days before the control group of patients. The efficacy of wound dialysis (pyosorbtion) was equal to 18%.

The article presents original experience with use of undecanoate (nebido, BayerHealthcare Pharmaceuticals, Germany) in androgenic testosteron replacement therapy in males with hypogonadism. Prospective studies of nebido efficacy were made in males with vein-occlusive erectile dysfunction (n = 20), chronic pelvic pain syndrome (n = 77), metabolic syndrome (n = 170). Retrospective studies assessed efficacy of nebido monotherapy in patients with erectile dysfunction and hypogonadism (n = 34), hematological and urological safety of the drug (n = 40). ⋯ Thus, nebido is highly effective in sexual dysfunction and other somatic disorders caused by hypogonadism. Nebido does not induce severe side effects, but clinically significant rise of PSA level requires treatment discontinuation and more careful urological examination. In view of nebido ability to suppress spermatogenesis, the drug should not be used in reproductively active men.

General and endoscopic examinations of 25 patients aged 40-48 years with chronic pelvic pain syndrome (CPPS) revealed combination of two urogenital diseases: interstitial cystitis and adenomyosis of the second-third degree. The complex treatment including a course of lavomax immunocorrection relieved CPPS in 72% (18) patients, reduced the number of recurrences of chronic pelvic diseases in 16% (4) patients. The therapeutic complex proposed may serve an alternative to surgical treatment of adenomyosis of the second-third degree which is uncomplicated with posthemorrhagic iron-deficiency anemia, manifests with pain only and associated with interstitial cystitis.

Prostatic surgery is often accompanied with increased intra- and postoperative bleeding which is caused, among other reasons, by enhancement of general and local fibrinolysis due to mechanical impact on prostatic tissue in operation. We proposed to inhibit enhanced fibrinolysis with preparation of tranexamic acid tranexam and tried it in 53 patients with prostatic adenoma undergoing transurethral prostatic resection (TUR). The comparison of bleeding severity in surgery with and without tranexam administration before, in and early after prostatic TUR showed that tranexam significantly lowers blood loss.
