AngioJet rheolytic thrombectomy versus local intrapulmonary thrombolysis in massive pulmonary embolism: a retrospective data analysis.
To compare the efficacy of full-dose local intrapulmonary thrombolysis (LIT) versus AngioJet rheolytic thrombectomy (ART) in the treatment of massive pulmonary embolism. ⋯ By accelerating the fragmentation of thrombus, ART plus adjunctive low-dose urokinase seems to be more rapidly effective compared to LIT. ART achieves both rapid cardiovascular relief and reduces the dose of thrombolytic agent necessary in patients with massive pulmonary embolism.

Sexual dysfunction after conventional and endovascular AAA repair: results of the DREAM trial.
To assess sexual function in the first postoperative year after elective endovascular aneurysm repair (EVAR) and open repair (OR) of abdominal aortic aneurysm (AAA). ⋯ EVAR and open elective AAA repair both have an impact on sexual function in the early postoperative period. After EVAR, recovery to preoperative levels is faster than after open repair, but at 3 months, sexual dysfunction levels are similar in both groups.

Thrombolytic therapy has been available for the last 5 decades, but the modern era of thrombolysis began in the early 1990s, with the execution of 3 multicenter trials designed to compare this potentially less invasive therapy to the then standard of care for acute limb ischemia, open surgical revascularization. Even with the development of several bio-engineered lytic agents, the major risk of thrombolytic therapy continues to be bleeding complications. Nevertheless, data exist to suggest that thrombolysis should be considered as an adjunct to open surgery, percutaneous interventions, or, occasionally, as sole therapy for acute vascular occlusion. This review summarizes the developmental milestones in the history of thrombolysis and reviews data supporting its use in acute arterial occlusions.

To report the use of a transradial approach to coil embolization of an intracranial aneurysm in a morbidly obese patient. ⋯ The radial artery appears to be a suitable route for access to the intracranial vessels when the femoral artery is not available.

To report the technique of carotid endarterectomy (CEA) combined with retrograde balloon angioplasty and stenting of proximal "tandem" lesions in the supra-aortic trunk. ⋯ Intraoperative innominate or left CCA balloon angioplasty/stenting combined with carotid endarterectomy is safe, effective, and durable.
