Insomnia severity is an indicator of suicidal ideation during a depression clinical trial.
Insomnia has been linked to suicidal ideas and suicide death in cross-sectional and longitudinal population-based studies. A link between insomnia and suicide has not been previously examined in the setting of a clinical trial. Herein we describe the relationship between insomnia and suicidal thinking during the course of a clinical trial for depression with insomnia. ⋯ The results support the concept that insomnia may be a useful indicator for suicidal ideation and now extend this idea into clinical trials. Insomnia remains an independent indicator of suicidal ideation, even taking into account the core symptoms of depression such as depressed mood and anhedonia. The complaint of insomnia during a depression clinical trial might indicate that more direct questioning about suicide is warranted.

Modafinil reverses hypoexcitability of the motor cortex in narcoleptic patients: a TMS study.
Although many animal and human studies have been performed, the exact mechanisms of action whereby modafinil promotes wakefulness are still not completely understood. We aimed to investigate the functional effects of modafinil on motor cortex excitability in patients with narcolepsy by means of transcranial magnetic stimulation (TMS) techniques. ⋯ This represents the first report investigating effects of modafinil on cortical excitability in human narcolepsy. Since SICI is thought to be directly related to GABA(A) intracortical inhibitory activity, we demonstrated that the dose of modafinil that induces a satisfactory wakefulness-promoting response in narcoleptic patients also causes decrease in GABAergic transmission.

The effect of modafinil on cortical excitability in patients with narcolepsy: a randomized, placebo-controlled, crossover study.
To investigate the effect of modafinil on cortical excitability in patients with narcolepsy using transcranial magnetic stimulation (TMS). ⋯ Narcolepsy patients with cataplexy showed decreased cortical excitability than normal healthy controls. Single dose modafinil significantly increased motor excitability in narcolepsy patients but had no effect in healthy controls.

Electronic media have often been considered to have a negative impact on the sleep of children and adolescents, but there are no comprehensive reviews of research in this area. The present study identified 36 papers that have investigated the relationship between sleep and electronic media in school-aged children and adolescents, including television viewing, use of computers, electronic gaming, and/or the internet, mobile telephones, and music. Many variables have been investigated across these studies, although delayed bedtime and shorter total sleep time have been found to be most consistently related to media use. A model of the mechanisms by which media use may affect sleep is presented and discussed as a vehicle for future research.

Ambulatory blood pressure before and after adenotonsillectomy in children with obstructive sleep apnea.
Hypertension is found to be associated with obstructive sleep apnea (OSA) in both children and adults. But data on the effect of blood pressure after adenotonsillectomy (AT) for children with OSA are limited and controversial. ⋯ In the current cohort of OSA children, 44% were cured of OSA and a significant decrease in overall diastolic blood pressure load in 24-h ambulatory blood pressure was achieved after adenotonsillectomy for children with OSA. But hypertension may persist or even occur in those previously normotensive children despite the improvement in AHI. Persistence of OSA may be a risk factor and further study is required. Cure of OSA should not be assumed after AT and follow-up PSG should be performed together with 24-h ambulatory blood pressure monitoring. In light of the current findings, long term study of the blood pressure is warranted for children with OSA.
