Vertical rectus abdominis myocutaneous flap cover for lower abdomen, chest wall, groin and thigh defects following resection of malignant tumours.
Vertical rectus abdominis mycutaneous (VRAM) flap provides a reliable flap cover for large soft tissue defects of chest wall, torso, groin, perineum and thigh. It has been mainly used in trauma and benign conditions. Between January 1994 through January 1999, eight patients with locally advanced malignant tumors underwent radical resection and reconstruction using pedicled VRAM flap. ⋯ There patients received postoperative radiotherapy and chemotherapy and two patients received radiotherapy only. All the the patients are alive and free of local recurrence at mean follow up 32 months. Results of our study shows that VRAM flap is versatile and sturdy flap with a wide are of rotation and it can reach diverse anatomical sites like torso, chestwall, thigh and perineum to cover large defects following radical resection for tumors.

Palliative treatment of advanced non small cell lung cancer with weekly fraction radiotherapy.
Various hypofractionated radiotherapy schedules of radiotherapy have been tried in patients of advanced NSCLC with no conclusive superiority of either a more protracted low dose per fraction schedule or a rapid course of large fraction size. Present study evaluates the role of weekly high dose radiotherapy for the palliation of various intrathoracic symptoms like cough, chest pain, hemoptysis, dyspnoea. A total of 47 patients of advanced non-small cell lung cancer were treated with four weekly fraction of 5 Gy to a total dose of 20 Gy. ⋯ Complete relief from dyspnoea was seen in only 5/12 (42%) patients whereas 2/12 (17%) patients had partial response. The present regime of 20 Gy in four weekly fractions was well tolerated by the patients without any identifiable acute toxicity and was simple and convenient to the patients, who traverse a long distance for radiotherapy in India. To conclude, weekly schedule af EBRT provide effective palliation of various intrathoracic symptoms in the patients with advanced NSCLC.

Review Case ReportsThe management of neuropathic pain in cancer: clinical guidelines for the use of adjuvant analgesics.
Neuropathic pain is seen in a third of cancer patients and is not always responsive to traditional analgesics. We describe practical guidelines for the use antidepressants and anticonvulsants as adjuvant analgesics in such situations. Newer adjuvant analgesics, interventional procedures and options for the management of pain emergencies, are also briefly outlined.

Extensive intraduct component in invasive duct carcinoma of the breast: prevalence and significance in a south Asian setting.
Extensive intraduct component (EIC) in invasive duct carcinoma is one of the main factors affecting local cancer recurrence and thereby a major consideration in breast conserving therapy. A retrospective study was undertaken to assess the prevalence of extensive intraductal component in a South Asian setting. 105 cases of invasive duct carcinoma reported at a University Pathology Department during a 75 month period from January 1992, were reviewed. 48.6 percent of all cases assessed had an intraductal component. 13.3 percent had an EIC. ⋯ The results of this study indicate that by virtue of its low prevalence. EIC in infiltrating ductal cancer is unlikely to be a major obstacle in the decision of breast conserving therapy in the South Asian region.

Case ReportsCongenital plexiform neurofibroma with a sarcomatous nodule in a three month old child.
Congenital plexiform neurofibroma is regarded as pathognomonic of neurofibromatosis (NF) especially when it is large and involves a major nerve trunk with changes of elephantiasis neuromatosa. Only very rarely malignant changes have been reported in cases who have NF for less than 5 years. 'Borderline' lesions are seen especially in patients with NF. ⋯ A case of 3 month child with congenital plexiform neurofibroma involving neck with elephantiasis neuromatosa with sarcomatous nodule has been described in the present article. The criteria for malignancy in nerve sheath tumors have also been discussed.
