Comparison of parenteral promethazine versus midazolam effect as a preoperative medication on postoperative nausea and vomiting after appendectomy.
Postoperative nausea and vomiting is a high unpleasant symptom in surgical procedures. This study compares the effectiveness of midazolam with promethazine as multipurpose drug in postoperative nausea and vomiting as a preoperative and anesthetic medication after appendectomy. Methods : This randomized, double - blind clinical trial study was performed in patients who underwent appendectomy. These patients randomly received either placebo or midazolam or promethazine 5 minutes before appendectomy. All episodes of vomiting and nausea within 24 hours after surgery were recorded. The study population (N=75) comprised 3 comparable subgroups (N=25 in each group). A standard general anesthetic technique and postoperative analgesia were used. The data were collected and analyzed. ⋯ Our study suggests that midazolam can be used as multipurpose drugs in postoperative nausea and vomiting as a preoperative medication after appendectomy and treatment using midazolam for anti-emetic, prophylaxis provide a similar effect compared to promethazine in the present study.

Transcranial doppler sonography diagnostic value for the cerebral flow velocity changes in the interictal phase of classic migraine.
An imbalance of the cerebrovascular response during functional activation of the brain has been postulated as a factor in the pathophysiology of migraine. The purpose of this study was to determine the transcranial doppler sonography (TDS) diagnostic value for the cerebral flow velocity changes in the interictal phase of classic migraine. ⋯ This stimulation we found altered cerebral vasomotor reactivity in the interictal phase in migraineurs with visual aura. This seemed to be an unavoidable hindrance for the wider implementation of functional TCD in diagnostic work up of migraine patients.

Case ReportsTracheoesophageal fistula following disc battery ingestion and foreign body impaction.
Ingestion of foreign bodies may result in the formation of a tracheoesophageal fistula (TEF), which causes severe morbidity in children. We describe four cases of TEF, who underwent emergent surgery for repair. ⋯ Long-term impaction of foreign bodies may result in tracheoesophageal fistula. This complication may be seen earlier with alkaline disc batteries. Removal of these foreign bodies should be followed carefully for the diagnosis and treatment of these fistulas.

A case of endobronchial polyp mimicking congenital lobar emphysema in an infant.
Congenital lobar emphysema, also called infantile lobar emphysema, is a respiratory disease that occurs in infants when air enters the lungs and cannot leave easily. It results from cartilage deficiency and bronchomalacia causing distal air trapping and respiratory distress. In this study, we present a case of endobronchial polyp mimicking congenital lobar emphysema. ⋯ The findings of this case emphasize the value of bronchoscopy prior to lobectomy in cases of congenital lobar emphysema.
