Present evidence demonstrates that mechanical ventilation in patients with adult respiratory distress syndrome (ARDS) contributes to the ongoing pulmonary damage, a condition known as "ventilator lung". Data from various animal studies indicate that volume, rather than pressure, is probably the main culprit. ⋯ Moreover, there is an added risk of atelectasis, which can be prevented by the application of positive end-expiratory pressure (PEEP). The present study reviews the pathophysiological mechanisms by which mechanical ventilation is injurious to the lung, and attempts to outline an approach aimed at minimizing such damage.

Case Reports[Recurrent, life-threatening anaphylaxis as initial manifestation of cystic echinococcosis (granulosus) of the liver].
A 34-year-old, previously healthy Yugoslav developed relapsing generalized anaphylactic reactions associated with life-threatening laryngospasm. After successful life support with volume-replacement, epinephrine, antihistamines and steroids, clinical workup disclosed a slight tenderness in the right upper abdomen and sonography a polycystic tumor with coarse calcification within the wall. The presumptive diagnosis of cystic liver echinococcosis with spontaneous rupture of a hydatid cyst was confirmed surgically and serologically. This case history shows that liver echinococcosis should be considered in otherwise unexplained anaphylaxis.

Vertigo consists of a variety of syndromes and can be due to many etiologies. One of these causes is migraine, which in our experience is often overlooked, although migrainous vertigo is well known in the literature. Vertigo in migraine can occur as aura or during the headache phase, or independent of the attacks as aura without headache. ⋯ Most of the patients had several attacks of vertigo, some involving up to 30 episodes. To recognize migraine as a cause of vertigo has therapeutic implications. Most of our patients with vertigo and migraine showed a good response to antimigraine therapy.

Resuscitation outside the hospital setting is a topic of increasing interest and discussion. A satisfactory neurologic outcome after acute cardiac arrest depends on immediate basic and the advanced cardiac life support (BCLS + ACLS). ⋯ To secure rapid assistance there should be one single phone number (e.g. 144) for emergency calls throughout Switzerland. Professionalism in the workup of emergency calls and in first aid by specialized personnel produces good results not only in cardiac but also in trauma patients.

[Efficacy of adenosine triphosphate in terminating supraventricular tachycardia].
The efficacy of adenosine triphosphate (ATP) was tested in 23 patients suffering from supraventricular tachycardia; 10 patients had orthodromic circus movement tachycardia and 13 typical av nodal reentrant tachycardia. At a mean dose of 0.18 mg/kg all tachycardias were halted in less than 1 minute due to transient acute av nodal block followed by return to normal av conduction within 8 seconds. ⋯ One elderly subject had a short episode of atrial fibrillation after the acute av block. In conclusion, ATP is useful and safe for rapid termination of supraventricular tachycardias incorporating the av node in their circuit.
