Transcranial Doppler sonography (TCD) enables measurement of blood flow velocities in the basal intracerebral vessels. The most important applications of TCD in the ICU are the diagnosis and monitoring of vasospasms caused by subarachnoid hemorrhage after rupture of an aneurysm. ⋯ TCD lends itself as a valuable non invasive bedside-monitoring tool, which enables continuous or intermittent monitoring of intracerebral hemodynamics without strain for the patients. Disadvantages to be mentioned are the dependence of experienced examiners and the long lasting training phase required to achieve the requisite experience.

Case Reports[Acute upper airway obstruction caused by cervical hematoma as a complication of an internal jugular vein catheter].
4 cases of inadvertent puncture of the common carotid artery following unsuccessful puncture of the internal jugular vein leading to a massive cervical hematoma are described. In all cases, acute upper airway obstruction required immediate intubation.

Biliary obstruction and bacterial infection are the most common cause of cholangitis. By modern diagnostic techniques the biliary tract and the liver can be explored in great detail. Decompression of biliary obstruction may be achieved surgically, endoscopically and via the percutaneous transhepatic route, bacterial infections can be controlled by powerful antibiotics, thus preventing the progress of cholangitis to secondary biliary cirrhosis following decompression of biliary obstruction has been documented. In view of recent diagnostic and therapeutic progress it seems likely that the incidence of secondary biliary cirrhosis will further decline in the future.

[Angiotensin converting enzyem (ACE) - a blood chemistry parameter in the diagnosis of sarcoidosis].
Serum angiotensin converting enzyme (ACE) activity was studied in 50 patients with sarcoidosis (39 active, 11 inactive sarcoidosis), as well as in 50 control patients (34 chronic lung diseases, 9 Hodgkin-disease, 7 rheumatoid arthritis). There is a significant difference (p less than 0.001) of ACE-activity between sarcoidosis patients and controls, and between active (without steroid treatment) and inactive sarcoidosis. Steroid treatment apparently lowers ACE-activity in sarcoidosis, however, without evidence for clinical improvement. Increased ACE-activity was also found in a patient with primary biliary cirrhosis.

[Chronic hypophosphatemic osteopathy ("rachitis"). Clinico-osteological review].
Chronic hypophosphatemia is the most common type of "resistant" rickets. Bone deformities, dwarfism and X-chromosomal dominant heredity, are regular findings in this "inborn error" of phosphate metabolism. ⋯ Oral administration of phosphate seems to improve longitudinal growth. A summary of our experiences in 44 patients is presented.
