Dr Bruce Trapp and Dr Daniel Ontaneda speak to Laura Dormer, Commissioning Editor: Bruce D Trapp, PhD, is a Chair of the Department of Neurosciences at the Lerner Research Institute, Cleveland Clinic (OH, USA) and Professor of Molecular Medicine at Case Western Reserve University (OH, USA). Dr Trapp received his PhD from Loyola University Stritch School of Medicine in Chicago (IL, USA). Dr Trapp's research investigates the cause of neurological disability in multiple sclerosis patients, cellular mechanism of brain repair in neurodegenerative diseases and the molecular biology of myelination in the central and peripheral nervous systems. ⋯ Dr Ontaneda earned his MD from the Universidad Católica del Ecuador and MSc in clinical research from Case Western Reserve University. He completed a postdoctoral fellowship at Baylor College of Medicine, followed by neurology and neuroimmunology training at the Cleveland Clinic. His specialties include acute disseminated encephalomyelitis, Devic's disease (neuromyelitis optica), multiple sclerosis, neuroimmunology, optic neuritis and transverse myelitis.

Observational StudyEffect of tetrahydrocannabinol:cannabidiol oromucosal spray on activities of daily living in multiple sclerosis patients with resistant spasticity: a retrospective, observational study.
To examine evolution in activities of daily living (ADL) in patients with multiple sclerosis spasticity during long-term use of tetrahydrocannabinol (THC):cannabidiol (CBD) oromucosal spray. ⋯ In this pilot study, THC:CBD oromucosal spray maintained or improved aspects of daily functioning. Further study in a larger trial is warranted.

Understanding important issues in young-onset dementia care: the perspective of healthcare professionals.
Psychosocial research on the lived experiences of young-onset dementia patients and caregivers has identified salient issues about their care, however, views on care from the perspective of young-onset dementia healthcare professionals is less well known. The aim of this study was to investigate and identify important issues in young-onset dementia care provision from a healthcare provider perspective. ⋯ The findings are presented as a paradigm for holistic young-onset dementia care. The paradigm offers a framework for contemplating and evaluating the criteria and quality of young-onset dementia care.

ReviewDemographics and clinical characteristics of primary lateral sclerosis: case series and a review of literature.
Primary lateral sclerosis (PLS) is a form of motor neuron disease involving only upper motor neurons. In some patients presenting as PLS, the disease progresses to involve lower motor neurons and thereby converting to amyotrophic lateral sclerosis (ALS). However, pure forms of PLS do exist. Our aim was to study epidemiological and clinical characteristics of pure PLS patients treated at our neuromuscular clinic. ⋯ Though on review of literature there is no clear consensus about the existence of PLS as a distinct disease entity, we believe that there are rare cases of motor neuron disease with progressive upper motor neuron symptoms that throughout their course never convert to ALS. Our series highlights the demographic and clinical features of these patients and underscores the longer survival of these patients when compared with ALS.
